4. 4 Functions of Community Dementia Service Community Dementia Nurses Principle function is to provide ongoing support from diagnosis to end of life Provide proactive intervention to reduce escalation of need and to minimise crisis or admission to hospital To focus on facilitating supportive interagency and ‘community asset’ working with support for primary care, to include district nursing and GP’s

5. 5 Care Home In-reach Service To support care providers with the management of individuals with dementia, in particular challenging behavior, so the person can be maintained in their care setting for as long as possible Training for care home staff

6. 6 Specialist Memory Clinic Work to the standards of the Memory Clinic National Accreditation Programme to provide rapid specialist assessment and diagnosis of suspected dementia based on three appointment model.
