2 What do we know Our local JSNA tells us: That there are increasing numbers of people with and affected by dementia in Suffolk That there is a greater reliance on expensive crisis, acute interventions due to a lack of early intervention and support

3 What do we know People affected by dementia (including Family Carers) tell us: There is not enough early support or information about how to cope with the impact that dementia will have on their lives Services are complicated as well as being difficult to access and to navigate

4 What we want for Suffolk People with dementia and their family carers maintain a good quality of life for as long as possible. People with dementia and their family Carers live a good quality of life and remain independent at home for as long as possible. People with dementia and their family carers have their physical, mental, emotional health and care needs met. People with dementia are able to die in accordance with their previously expressed wishes. Family Carers feel supported throughout. Everyone has a crisis contingency and advanced care plan in place (Shared Care Planning) Services that: – Raise Awareness & Understanding – Provide Early Support & Intervention – Promote independence by Maintain Activities of Daily Living – Provide support and care in a Crisis & at the End of Life

5 How we will do this Commission integrated services for those affected by dementia and their family Carers. Develop and promote dementia-friendly communities and neighbourhood networks Provide support for Family Carers through the Wellbeing service. Commission Integrated Neighbourhood Teams & Specialist Dementia teams at the same time as bringing together key VCS providers

6 What needs to happen next Suffolk CCG’s and SCC are working together to develop a Business Case to confirm commissioning arrangements that will consider: – New service models and partners – How to reshape existing services to deliver earlier intervention and support – Joint commissioning and procurement options – How services will be made more sustainable and deliver better value for money In the interim Commissioners will continue to work with local providers to re-model existing services and resolve boundary issues within the current pathway.
