3 Stages of OT Process: Referral
Point at which initial request for services for particular client at a change in the degree and direction of services is made Self – referral Referred by a physician, another professional, or another agency

5 Screening process may involve: review of the client’s records
use of screening test interview with the client and caregiver observation of the client discussion of the client’s referral source determine whether further evaluation and OT intervention is warranted

Discharge plan – conforms to the personal goals and addresses the resources available in the community Discharge summary – summary of the status of the client’s functional level at the time of discharge, it may contain the plans for discharge, equipment and services recommended after discharge
