4 Your Business Transitions… What is your transition process? How do your clients/customers transition into your service, and out of your service? Have you made it easy/hard to transition? Have you ever had a conversation with those that you transition your clients/customers to? Have you ever asked your clients how that transition went?

Brought “all the players” to the table Took already approved agreement and expanded and defined roles of community partners Focuses on all children, ages prenatal-6 From 1992-2004, the Kentucky Early Childhood Transition Project facilitated a process to develop interagency agreements with First Steps providers and the local education agency (school districts) for children with identified special needs

It is designed as a resource for anyone going through, or helping someone with, a life change including people with disabilities who may have additional needs during times of transition. Nine key transition points were identified by the partners who built this site. They are listed on the left menu. Whereas each transition point involves its own specific challenges and considerations, there are also effective practices common to all. Therefore, within each transition point you will find content arranged according to a common outline, accessed by clicking on the transition point.
