Reportable Occurrences – Injury Notice Regulation 854 Sec. 21 (5)(i) (5) In addition to the occurrences referred to in section 53 of the Act, a notice in writing shall be given where, (i) an unexpected and uncontrolled run of material, water or slimes in excess of one cubic metre occurs that could have endangered a worker. 3

Runs of Material Incidents & Categories 2002-2011 Total Incidents- 100 *Runs of Muck Underground -56 Incidents *Incidents at Loading Pockets - 14 Incidents *Muck Falling Down the Shaft - 30 Incidents 4

Causes of Runs of Water Underground Overflowing sumps Punctured fill barricades Dust suppression sprays Spring run off Known wet passes Leaking fill lines Seepage from near by pits in spring Broken water lines 5

Causes of Run of Muck Underground Runs of water into ore pass Dumping wet sand fill into ore passes Impounded water in raise bore holes being reamed Malfunctioning chute gates Adding water to muck in passes that was sticking due to the presence of wet muck and fines Failing to pull the pass for a number of days 6

Causes of Muck Down the Shaft Operator not following proper procedure Faulty measuring device Clearing a hung up rock Manually activating the solenoid of the discharge door in the wrong compartment by the skip tender Bin probe not reset after repairs Defective relay put skip too low Hopper door staying open Arc gate not closing Transfer car cylinder failure Catch pit failure 7

Prevention of Incidents Relating to Water or Slime Material from Entering Passes Work should be stopped immediately A supervisor and all workers in the vicinity should be notified All affected work areas should be signed, barricaded against entry and evacuated The source of the water must be found and stopped as soon as possible All drain holes in a mine must be checked and cleaned on a regular basis A procedure to safely remove a build up of water should be established and reviewed with the workers involved A general policy, backed up with training and safety meetings, about managing the flow of water through a mine. Water used for washing and dust control must be monitored and controlled 8

Run of Materials Compliance & the IRS Regulation 854 Sec. 87 – Water Accumulation (1) A workplace in an underground mine shall, be kept free of the accumulation of water have a drainage system to pump excess water to a pumping system capable of pumping water to surface for disposal (2) Where accumulations of water are likely to be present, A borehole shall be drilled at least six meters ahead of the working face to protect against a sudden breakthrough of the water, and Precautions shall be taken to control the flow of water (3) A positive displacement water pump shall be equipped with a relief valve or system (4) Precautions shall be taken to guard against an accumulation of water in a chute or raise where the material may block drainage 9

Run of Materials Compliance & the IRS Regulation 854 Sec. 61 – Stockpiles of unconsolidated material Sec. 88 – Surface Mining Sec. 62.1 – Non-routine Hazardous Tasks 10

Run of Materials Training Requirements Regulation 854 Sec. 11 – Training (Common Core) Employers in the types of mines and mining plants described shall establish and maintain the training programs identified Basic Underground Hard Rock Miner– Common Core Program #770010 U0001.08 - Perform General Inspection Identify and report water in chutes and hang ups, by performing visual inspection; reporting deficiencies; and performing repairs including: diverting water and/or shutting off water hoses according to government legislation and company standards* 11

Run of Materials Training Requirements Regulation 854 Sec. 11 - Training - Common Core, Specialty Modules U0068a.06 - Perform Trackless Haulage – Operate Scoop Draw Point ….monitoring condition of muck flow and brow; monitoring hang-ups; handling explosives; ensuring water is un-impounded U0088.04 – Operate Skip (new operate loading pockets U0088) …monitoring water impoundment; inspecting sprinkler systems; locking & tagging; and repairing and/or reporting deficiencies U0088.05.monitoring muck flow, controlling tramp material & wet muck 12

Run of Materials Training Requirements Regulation 854 Section 11 – Training Common Core, Specialty Modules (continued) U0077.06 – Operate Underground Crusher Crush muck, by controlling dust, muck flow, muck level, wet muck, and tramp material hazards; identifying and responding to blasting agents; guarding and blasting; controlling other sources of water, including: catch pits, ground water, ditches, back filling and drilling operations; and repairing and /or reporting deficiencies according to government legislation, company standards and manufacturers specifications* 13

Run of Material Training Requirements Regulation 854 Section 11 – Training Common Core, Specialty Modules (continued) U0077.07 - Operate Underground Crusher …monitoring runs of muck; locking out and tagging, and repairing and/or reporting deficiencies according to government legislation and company standards U0077.08 – Operate Underground Crusher …guarding against hazards, including: water, runs of muck and tramp materials 14
