4 Employee Benefits – who is eligible?
All Employees who REGULARLY work 30 hours or more per week Most benefits start on the first day of the month following an employee’s first full month of employment

5 Medical Insurance Regence Blue Cross Blue Shield of Oregon
Employee only coverage is paid 100% by the School, but employees are responsible for the cost of adding ALL other dependents Plan type is PPO Regence Innova w/ $1,500 deductible but flat in-network co-pay for PCP and Specialists The plan renews on October 1, 2013

6 Medical Insurance Cost
Employee Only - $0.00 (100% paid by School) Employee + Spouse – $379.30 Employee + Child(ren) - $220.70 Employee + Family - $689.60 *All rates are per month and effective October 1, 2013

7 Principal Dental, Short-term Disability and Long-term Disability
Dental is a Voluntary Program Employees pays 100% of discounted premium for themselves and their dependants Employee Only - $40.79 Employee + Spouse – $87.12 Employee + Child(ren) - $90.94 Employee + Family - $140.28 *All rates are per month and effective October 1, 2013 STD is 100% paid by the School LTD is 100% paid by the School Can pay up to 60% of your monthly salary if you cannot work after 4 months, until you either get better, reach the limit of certain illnesses, or reach retirement age

8 Dental Insurance Cost Employee only - $40.79

9 Cafeteria Plan Vendor - Polestar Flexible Spending Account (FSA)

10 TIAA-CREF Retirement Savings 403(b) plan
Employee can deduct portion of paycheck into Account PRE-INCOME TAX (you still pay Medicare & Social Security) After ONE YEAR of full-time employment, the School will match deposits up to 3% of salary (Mon-Fri 5am-7pm and Sat. 6am-3pm PST)

11 Open Enrollment - Deadlines
Please signup to meet with me! Forms to complete and submit Benefit Overview Form. Tells me what benefits you want to change Medical Enrollment or Change form Dental Enrollment or Change form TIAA-CREF Enrollment or Change form Flexible Spending Account Form Direct Deposit Form W4 form (if you want to change tax withholding) All enrollment or change forms are due no later than Monday September 16, 2013 (new participants, adding or changing coverage, adding or deleting dependents)

12 Monthly Deadlines – The 20th of each Month
W-4 form – Changes for Tax Withholding Allowable insurance changes due to qualifying event Benefit Cancellations TIAA-CREF Retirement Account Deduction Changes Direct Deposit changes* *Direct Deposit additions and account changes can take over a month to complete. So file a Direct Deposit form as soon as possible.

13 Payroll Payroll period 26th – 25th and bridges 2 months
Payroll is monthly: Pay date is the last day of the month (or first day of following month if a weekend is involved) Distribution Options: 10 Months or 12 Months, starting in September Payroll Advances: For Emergencies only

14 Timesheets are now Electronic with ADP
Must be completed by ALL hourly employees: Teacher Assistants, Admin Assistants, Maintenance Staff, Employees who work less than 40 hours per week ALL employees will use ADP for “Time off Requests” if they are planned in advance “Time off Requests” that are call in sick or emergencies employees must use PDF form and it after the absence to their supervisor for approval “Time off Requests” for PD or school duties when you get substitute coverage employee must use PDF form and it in advance for supervisor approval
