1 Nursing Library Training using Sunrise Press data

2 Assign Student Staff member must be given permissions to be a Nurse or Health Plan Manager Students requiring a Health Plan need to be assigned to nurse prior to creating a health plan

3 Create Health Plan Select case Click on Health on the menu Click on Create New Health Plan

4 Create Health Plan Key in appropriate dates Click submit

5 Click in the text box and fill in the Assessment information according to your district’s guidelines. The items labeled with red text and an asterisk (*) are required information that must be filled in if they are not already done.

6 If you don’t know all of the information at this time, you may leave some of it blank, but you’ll need to come back later and fill it in before you can finalize the document. Click the Submit button, a message is displayed indicating that the plan information was saved.

7 Add Medical Diagnosis On bottom portion of the Menu Bar, click Medical Diagnosis. Click the Add New Medical Diagnosis link.

8 Medical Diagnosis The Health Plan Medical Diagnosis Definition page is displayed.

9 Medical Diagnosis Click on an item in the ICD-9/DSM-IV Area box to select it. The list in the ICD-9/DSM-IV Code box is updated with the relevant items for the ICD-9/DSM-IV Area selected. Click on an item in the ICD-9/DSM-IV Code box to select it.

10 Medical Diagnosis Click the Show Descriptions button. A description of the selected Code item will be displayed. You must click the Show Descriptions button to update the description if you click a different item in the boxes.

11 Medical Diagnosis After all medical diagnoses are selected for that student, there will be options to re-sequence the diagnoses if you wish to have the items appear in a particular order (i.e., severity, code, alphabetical), depending on your district’s guidelines. Click in the sequence number fields for each Medical Diagnosis and renumber them as desired.

12 Medical Diagnosis The list is reordered according to the sequence numbers typed. Click the Submit Sequence Changes button. If you enter sequence numbers that are not numbered consecutively starting with 1, you will receive a warning message alerting you. Duplicate and out of sequence numbers ARE allowed by the system, but the Health Plan cannot be finalized unless they are in sequence.

13 Nursing Diagnosis On Menu Bar, click Nursing Diagnosis. Click the Add New Nursing Diagnosis link.

14 Nursing Diagnosis The Search Library for Nursing Diagnosis page is displayed. Click the Nursing Area drop-down menu and select a Nursing Area as appropriate. This menu provides a customized list of Nursing Areas used in your district.

15 Nursing Diagnosis If no Search Patterns are entered, all of the items in the selected Nursing Area will be found. Based on Medical Diagnoses, the first Nursing Area selected will be Anaphylaxis/Allergy Click the Submit button.

16 Nursing Diagnosis The Nursing Diagnosis Search Results page is displayed showing all of the items matching the search criteria. Click in the checkboxes to select the Nursing Diagnoses you wish to use. Click the Submit button.

17 Nursing Diagnosis The Nursing Diagnosis List is displayed with the items you just added. Sequence numbers may be changed in the same way that they are for Medical Diagnosis List.

18 Nursing Diagnosis You may wish to have the items appear in a particular order (i.e., severity, code, alphabetical), depending on your district’s guidelines. Click in the sequence number fields for each Nursing Diagnosis and renumber them.

20 Nursing Diagnosis The Health Plan Goal Definition page is displayed. You may edit the description as necessary, and any changes you make will apply only to this Health Plan and will not affect the description in the library. Click the Submit and View Goals button

21 Nursing Diagnosis The changes are saved and the Health Plan Goal List is displayed. Now, Juli has one goal for the Nursing Area: Anaphylaxis/Allergy and Diagnosis of Ineffective Breathing Pattern. You may click delete to remove the goal. Click Edit to Add Interventions and Outcomes for that Goal.

22 Nursing Diagnosis Edit text if necessary Click Add Interventions and Expected Outcomes from Library

23 Nursing Diagnosis Click Add New Nursing Goal from Library

24 Nursing Diagnosis Select one or more goals Click Submit

25 Nursing Diagnosis Click edit next to Goal to Add Interventions and Outcomes for that Goal

26 Nursing Diagnosis Select one or more interventions and expected outcomes for that goal Click Submit

27 Nursing Diagnosis Selected interventions and expected outcomes were successfully entered should display Click Edit next to Intervention or Outcome to change text If necessary, click to add another intervention or outcome or delete to remove an intervention or outcome

28 Nursing Diagnosis If OK, click Submit Click Nursing Diagnosis on menu bar to repeat the process on the next Goal When done adding interventions and outcomes for each goal, add another diagnosis or add notes and Health Plan Guardians

29 Nursing Diagnosis Do a Check Completeness Check Correct items that are missing by clicking Edit next to the item

30 Nursing Diagnosis Click edit next to Nursing Goal #2 for Nursing Diagnosis #1 It requires at least one intervention and one outcome

31 Nursing Diagnosis Click add nursing goal from library Or click add new nursing goal if you want to create your own text

32 Nursing Diagnosis Create your own goal text Click Submit when done

33 Nursing Diagnosis After you have made all the necessary changes Check completeness should come back clean and display that the Health Plan is complete You may make this Health Plan final or click Not Yet if you need to wait for approval

34 Nursing Diagnosis Click Print PDF on Menu bar to view Diagnosis, Goals, Interventions and Outcomes for Juli. Correct Health Plan by edit plan as necessary

35 Nursing Diagnosis To view all assigned health plans click View Health Plans on main menu bar

36 Creating an Emergency Care Plan As Health Plan Manager select student to create ECP using View Health Plans on Menu

37 Creating an Emergency Care Plan Click on Create New Emergency Care Plan link

38 Nursing Assessment (optional) Fill in nursing assessment of child’s condition

39 Symptoms Click on Symptoms link on Emergency Care Plan menu. Symptoms/actions can be added manually or via the Nursing Library. Also, if a symptom is deleted from the plan, all actions associated with it will also be deleted.

40 Symptoms Fill in symptom information

41 Actions Click on Add new Action link after Symptom has been created

42 Actions Fill in action information
