1 CCHD Grant Application Using the Online System

2 Sign In to the Online System Enter you complete e-mail address If this is your first time using CCHD’s online application, click “I am a new online applicant” and click Continue. If you have set up an account in the past, enter your password, then click Continue.

3 Create Your Account If you clicked on “I am a new online applicant,” this screen will appear. Enter your complete e-mail address.Enter and confirm your password. Your password is case sensitive, write it down so you’ll remember it!

4 Working with the Online Application Use the links to learn more about required attachments and criteria for funding or to contact CCHD staff with a question. PLEASE NOTE: Clicking the “Exit” link will NOT allow you to save your work.

5 Working with the Online Application Click on the numbers to go to different pages of the application To attach documents click on the last underlined page number Click “Review My Application” before you submit, to verify that you have completed the required fields Click “Printer Friendly Version” to open an HTML document of the entire application. We recommend doing this if you are going to print a hard copy.

6 Navigating through the Online Application To save a draft of your application which you can return later, click “Save & Finish Later” To move to the next page and continue working on the application, click “Next.”

7 Uploading Attachments If the application requires the submission of attachments, instructions will be found on the last page of the application. Templates for the attachments are accessed by clicking the Attachment Forms link at the top of the page. To upload attachments: Remember to click on last underlined page number to access this page 1 st – Choose the type of attachment you are uploading from the drop down menu. 2 nd – Click Browse to look in your computer’s directory to find the file you would like to attach. 3 rd – Click Upload. 4 th – Repeat process for each attachment. Business plans, press articles and other required attachments which are not available in electronic format, may be submitted via snail mail.

8 Saving and Submitting Your Application Once you have completed your application, proof read it and uploaded any required attachments, you are ready to “Review and Submit” your application. If you would like to save the application and return to review it later, click “Save & Finish Later.” PLEASE NOTE: Once you submit your application, you will NOT be able to make any changes. Be sure to take the time to proof read and review it.

9 Your Account Page Any time you log out of the application by saving or submitting your work, you will be directed to your account page. This page lists all of your pending and submitted applications.
