3 I. Program Overview California law authorizes DLC and the four OPOs to register an individual using an internet- connected device By collecting information from an individual By collecting information from a driver license or DMV-issued ID card

4 A. User Requirements Each user must: Be an active Donate Life Ambassador Pass a criminal background check Complete eRegistration training Read and electronically sign: Confidentiality Agreement Acceptable Use Policy

5 B. Approved Devices The Donate Life California eRegistration web application can be accessed using an internet connected web browser and is permitted to be used on a personal device.

6 C. Confidentiality Agreement & Acceptable Use Policy All users are required to agree and sign the agreement the first time they login to the eRegistration application It will be required every year on the anniversary of the first login The information is kept confidential and secure by Donate Life California Do not shared login information Each user has a unique username and password

7 Ensure the Wi-Fi or cell signal is activated Using your web browser, enter in the URL: https://m.donatelifeca.org Enter in your assigned username and password II. Using Web Application

8 Read and sign the Confidentiality Agreement Read and sign the Acceptable Use Policy Agreements Check the box and then tap “Accept”

9 Event/Campaign Name Enter in the name of the event as well as the date.

10 Home Screen Read or have the individual read the information on the screen. If the individual agrees, tap “Accept.” After acknowledgment, tap “Accept”

11 The four navigation buttons will take you to the HOME screen, SETTINGS to change languages, PRIVACY, for the privacy policy and EXIT, to quit the web application. Home, Privacy and Exit Home Privac y Exi t Settin gs

12 Information Screen There are two ways to enter in information: 1.SCAN the back of a driver license or ID card with the device’s camera 2.ENTER the personal information into the data fields

13 Tap the blue “Scan” button. It will ask you to choose an action” Scan Tap “Scan” Then select “Camera”

14 SCAN Point the camera to the 2D barcode on the back of the driver license or ID card 2D barcode Magnetic strip

15 SCAN If it does not have a 2D barcode on the back of the driver license or ID card, you may use the SWIPE or ENTER in the data 2D barcode

16 SCAN Once the picture is take and uploaded, you will see the progress bar reading the information Progress bar

17 SCAN If unsuccessful, an error message will appear Error message

18 ENTER IN DATA To enter in the individual’s information, tap on each field and type using the on-screen keyboard.

19 Reviewing the Information Once the information is entered, review the information to make sure all required fields are entered. Then have the individual confirm the information.

20 Reviewing the Information If the data is incorrect or tap on the text area Using the screen keyboard, enter the correct information.

21 Reviewing the Information When all the information is correct, have the individual read the consent statement and tap “Accept.”

22 Signature Screen Ask the individual to provide a signature using their finger.

23 Signature Screen If the individual is under the age of 18, it will ask if a parent is available to sign.

24 Signature Screen If yes, it will ask for the first and last name of the parent.

25 Signature Screen It will also ask for a parent signature

26 Signature Screen If a parent is not available, tap “no.”

27 Receipt Screen The individual will be asked if they would like an email receipt.

28 Receipt Screen The individual will be asked if they would like an email receipt.

29 Receipt Screen If yes, select “yes” and enter in the email address using the screen’s keyboard.

30 Receipt Screen If no, select “no.”

31 Confirmation Screen After a successful registration, a “Registration successful!” message will appear.

32 Confirmation Screen To continue on to register the next individual, tap “Home.”

33 Confirmation Screen If you are done using the application, be sure to Exit by tap the icon at the top right of the screen. Exi t

34 III. Privacy Policy Donate Life California has adopted and adheres to stringent security standards designed to protect your personal information from unauthorized access or disclosure. Among the safeguards that we have developed for our site are administrative, physical and technical barriers that together protect the information stored at this site. No information is ever stored in the application, to the device or device memory.
