6 Forms A-D The following forms, provided by your teacher, must be part of your proposal: Form A: Cover Page Form B: QLI Project Acknowledgements Form C: Research Review Committee Checklist QLI Survey

7 Important Reminders Due: Friday, April 16, 2015 6-15 pages in length
Double-spaced, 12 point font, Times New Roman or Calibri Type all forms 8 different citations in your paper One must be a primary source References/bibliography section Cite each source correctly (MLA or APA Style)

8 Checklist for QLI Proposals
Have 2 people proofread writing for grammar/spelling errors Name only on Forms A, B, C, & D and on no other page Project Acknowledgments (Form B) Title page with a short, descriptive title Place title on the upper-right-hand corner of every page Put page # at the bottom of each page

9 Checklist for QLI Proposals, continued
Cite 8 different sources in the body using MLA or APA style List of sources on the References Page. The sources should match those cited in Proposal Arrange proposal in the correct order Attach completed QLI Survey (Form C) Two copies of proposal, one to submit and one to keep.

10 Final Checkpoints Body of Proposal no less than 6 & no more than 15 pages. Number the pages - Summary page is number one. Background Section not more than 4-5 pages. If research involved people, Research Review Committee Checklist (Form C) must be included with paper
