6  Would you welcome this project?  How it works  We provide you with evaluation questionnaire (tailored to your service)  You ensure they are sent quarterly to staff, service users, families, professionals  All feedback is addressed to HCPA and is collected and analysed  A report is provided to you  A coaching session is arranged with the manager/owner to discuss outcomes  The independent evaluation feedback service is designed to provide you with feedback to dramatically improve the quality and consistency of your feedback by increasing response levels (because the replies are not sent directly to the care provider and therefore seen as truly anonymous). 9  The Government has made it clear that better integration of care must be at the heart of the reformed health system.  The Nuffield Trust and The King’s Fund were asked by the Department of Health to support the development of a national strategy for the promotion of integrated care.  The findings have been submitted to the DH and the NHS Future Forum.  Results “We need to move beyond arguing for integration to making it happen, whilst exploring the barriers”NHS Future Forum

10  Government policy should be founded on a clear, ambitious and measurable goal to improve the experience of patients and service users and to be delivered by a defined date.  Setting an ambitious goal to improve patient experience should be reinforced by enhanced guarantees to patients with complex needs. These guarantees would include an entitlement to an agreed care plan, a named case manager responsible for co- ordinating care, and access to telehealth and telecare and a personal health budget where appropriate.  Change must be implemented at scale and pace. This will require work across large populations at a city- and county-wide level. There should be flexibility to take forward different approaches in different areas and to evaluate the impact, with the emphasis being on people with complex needs.

11  We are going to start our day with a look at Clinical Commissioning Groups in Hertfordshire Herts Valleys Clinical Commissioning Group - covers the geographical areas of Dacorum, Hertsmere, St Albans, Three Rivers and Watford. East & North Herts Clinical Commissioning Group is made up of six locality groups. Lower Lea Valley, North Herts, Stevenage, Stort Valley and Villages, Lea Valley, Welwyn and Hatfield  A key priority is to deliver more care to people outside of hospital, closer to where they live and within their own homes, wherever that may be.

12  Clinical commissioning groups (CCGs) are NHS organisations set up by the Health and Social Care Act 2012 to organise the delivery of NHS services in England.Health and Social Care Act 2012NHS  CCGs are clinically led groups that include all of the GP groups in their geographical area. The aim of this is to give GPs and other clinicians the power to influence commissioning decisions for their patients. GP  CCGs will be overseen by NHS England. These structures will manage primary care commissioning, including holding the NHS Contracts for GP practices NHS.NHS England

16  CCGs operate by commissioning (or buying) healthcare services including:  Elective hospital care  Rehabilitation care  Urgent and emergency care  Most community health services  Mental health and learning disability services  Clinical commissioning groups work with patients and healthcare professionals and in partnership with local communities and local authorities.
