4 Enrolling at Soquel High School  You must enroll at Soquel High as we are a different district – Santa Cruz City Schools  Fill out and complete all four forms  Enrollment form (Blue)  Freshmen Course Requests (Green)  Student Health History (Green)  Home Language Survey (White)  Signatures required on ALL forms

5 Turn in Enrollment forms to your Counselor  New Brighton Middle School  Counselors visit on Wednesday, March 26  Shoreline Middle School  Counselors visit on Tuesday, March 18  Privates and Charters  Call Counseling Assistant at 429-3909 x 132 for enrollment appointment with counselor
