9 10/6/2015 9 NEAR MISS INCIDENT  A near miss incident Results in neither injury or property damage but has the potential to cause either or both Occurs due to an unsafe act or condition Requires corrective action to prevent recurrence Examples: A box sitting on an overhead shelf isn’t pushed back all the way when it suddenly falls and misses the employee standing below by a few feet Cords running across an area that people walk – trip hazard Icy sidewalks Wet / slippery floors Broken outlet covers

10 10/6/2015 10 EMPLOYEE RESPONSIBILITY  Report ALL near miss incidents verbally to your supervisor or director IMMEDIATELY Unsafe Act Immediately inform co-worker action is unsafe Unsafe Condition Remove yourself and others from immediate danger  Submit a “Near Miss Incident” Report Immediately  Participate in the incident investigation

11 10/6/2015 11 EMPLOYER RESPONSIBILITY  Inform appropriate people if immediate danger  Complete and submit “Near Miss Incident” Report Including: Investigation of incident Recommendation of corrective action Consider disciplinary action if/when appropriate Distribute report appropriately  Implementation of corrective action  Train and/or inform employees

12 10/6/2015 12 WORK-RELATED ACCIDENT  ANY unplanned, undesired event not necessarily resulting in injury, but resulting in damage to property and/or interruption of the activity in progress Example: Vehicle damage due to deer hit

13 10/6/2015 13 WORK-RELATED INJURY  ANY injury arising out of and in the course of employment “Arising out of” Must be a connection between work and injury “In the course of” Time, place and circumstances of injury

14 10/6/2015 14 EMPLOYEE RESPONSIBILITY  Report ALL accidents AND/OR injuries to your supervisor IMMEDIATELY No matter how minor or insignificant they may seem  Report ALL accidents AND/OR injuries to the fiscal officer IMMEDIATELY OSHA Regulations mandate the timeframe in which an employer must report an injury

15 10/6/2015 15 EMPLOYEE RESPONSIBILITY  Request medical attention if necessary Return “Report of Work Ability” form to fiscal officer Inform Supervisor of any work restriction FOLLOW WORK RESTRICTIONS!!!  All follow-up appointments require a “Report of Work Ability” form to be filled out by doctor  Keep supervisor/ fiscal officer informed of on- going restrictions  Participate in the accident investigation

16 10/6/2015 16 EMPLOYER RESPONSIBILITY  Post-Injury 1 st Report of Injury (as described by employee) Call 911 if life threatening injury Have employee make a doctors appointment if warranted “Report of Work Ability” forms to employee FMLA forms to employee when needed Copies of all forms to MCIT  In-house Investigation Accident Investigation Report Implementation of corrective action  Post-Injury Strategy Keep in contact with the employee Work with employee regarding any questions, or concerns they may have Return to work issues Compensation issues, etc.

17 10/6/2015 17 EMPLOYER RESPONSIBILITY  In-house Accident Investigation Accident Investigation Report Who was involved? Any witnesses? What happened? What was the activities before the incident occurred? When (in detail) did events occur? Where (in detail) did the event take place? How or why did the event take place? What equipment was involved? Implementation of corrective action Post-Injury Strategy Keep in contact with the employee Work with employee regarding any questions, or concerns they may have Return to work issues Compensation issues, etc.
