OHIO University Student Health Insurance Policy All students are required to have an active health insurance policy while attending OHIO University Your health insurance is provided by a commercial company named United Healthcare Student Resources. Your insurance is valid July 26, 2015 through August 15, 2015

4. INSURANCE ID CARDS Insurance Identification Cards are provided to you in your insurance information packet You will need your identification card to: – Receive Medical Treatment – Purchase medications from a pharmacy at a reduced rate

5. WHAT DOES THE INSURANCE PAY FOR? The health insurance benefits at Campus Care are for: An office visit to see a doctor if you are sick or injured – You will owe a $15.00 copay to see a doctor at Campus Care Laboratory testing for diagnostic purposes X-Rays (Radiology) Reduced rate for medications from the Campus Care pharmacy – You will owe $5.00-$35.00 for medications purchased at the Campus Care Pharmacy

6. WHAT DOES THE INSURANCE NOT PAY FOR? The Health insurance will NOT pay for: An office visit to see a doctor when you are not sick, or do not have an injury or illness The $15.00 copay to see a doctor at Campus Care Dental Treatment Vision Screenings Medical treatment for items listed on pages 3-4 of the policy summary; PLEASE REVIEW
