What’s required in an approved training agreement? www.rpmoll.com Student’s personal information: name, address, telephone, birth date, emergency contact School name, address, telephone (2x), contact person Employer name, address, telephone, contact person List of student, employer, and school responsibilities Date of appropriate safety instruction (employer or school provided) Beginning & end dates of agreement Daily hours to be worked, including beginning & end times Verification of workman’s compensation & general liability insurance Signatures of: student, parent/guardian, employer, coordinator, & principal or designee

What’s required in an approved training agreement? www.rpmoll.com The following items are required, but will not be reviewed by the auditor Beginning rate of pay if paid work based learning experience Statement of assurance from the employer of non-discrimination Statement of assurance from the school district of compliance with non- discrimination

What’s required in an approved training plan? www.rpmoll.com For unpaid learners, new specific skills listed for each 45 hours of the placement Identify related academic courses (non-CTE & non-SE only) pupil currently or previously enrolled in Signatures of: student, parent/guardian, employer, coordinator, & principal or designee The following items are required, but will not be reviewed by the auditor Teacher verification that the pupil’s placement relates to educational development plan. For CTE students, verification must be made by the vocationally certified instructor List of performance elements that contribute to pupil’s progress & assessment
