The process of getting young people, families, providers, services and other key stakeholders together in one room to understand what we all want for young people is vital. 5  With young people, families and all key agencies that need to work together to help young people with SEND achieve better outcomes  Focussed on life chances  Shared understanding of what is positive and possible and what works  Use the pathways to getting a life to carry out a gap analysis of what is working, what is missing and agree priorities for commissioning and joint commissioning  This will help to develop the statutory requirements of the Local offer

7 Start from the beginning Where do young people and families get good information, advice and support that helps them to achieve the preparing for adulthood outcomes ? How do they hear from other people about what has worked for them ? Do people hear from employers ? Year 9 preparing for adulthood review

8  Person centred outcome focussed EHC planning and reviews  Personal budgets across health, education and social care  New preparing for adulthood review

10 At least - from Year 9 must focus on preparation for adulthood Annual reviews from Year 9 must include a focus on:  planning of support for SEN, health needs and social care needs to meet the planned outcomes around employment, independent living and housing, friends and community participation and health and wellbeing;  the best post-16 provision to help achieve career aspirations and planned outcomes options and choices for next phase of education; e.g. invite representatives of post 16 institutions to review meetings;  the new legislation requires that young people’s views are taken account of and from age 16 are paramount;  planning of transition to adult social and health services where necessary. 

12 Develop post 16 provision and support that leads to good life outcomes - Hertfordshire Multi agency (Education, health and social care) planning and commissioning for post 16 options and support Early intervention – plan for needs and aspirations from year 9 Working with adult health, social care, housing, employment and educating partners to develop pathways for young people – e.g. supported living and short breaks options with local FE college

14  Use information from local offer to inform gaps in provision  Use information from EHC plans to forecast demand  Identify outcomes you want to achieve  Made full use of joint commissioning duties
