12 Online Registration Download 3-page Physical Exam form and return to AD when complete. Have your health insurance and car insurance information, as well as your 7-digit student ID #, available.

13 Uniform Complaint Protocol If a problem in a sport arises, please follow these steps: 1.Athlete sets up discussion with coach 2.Parent, athlete, and coach meet 3.Parent or athlete presents written description (hard copy or e-mail) of issue to Athletic Director 4.Parent, athlete, coach, and AD meet 5.Parent, athlete, coach, AD, and Principal meet 6.Principal contact with District Office for resolution
