4 Purpose of the Work Plan Time for reflection at the end of each learning session Guide for action between learning sessions Documentation of steps completed and incomplete Just a template- edit or create your own if you wish

5 Fill out Section 1 of Work Plan For each CHA task/activity, consider:  Have we done this already? If so, did we document what was done? How might we change it for the future?  Is Technical Assistance needed to accomplish this task?  Are these goals realistic and attainable?

6 Develop A Plan Define the Community Establish the CHA Team Identify a target date for distribution of CHA Compare and contrast models Identify factors that predict success and quality

7 Define “Community” Has this been done already? Who completed it or will complete it? What is the timeline? Is your community defined geographically?  A city? County? Multiple counties? Are there groups in the community that are vulnerable?

8 Establish the Team Will you partner with a local hospital? Other health departments? Are there structures in place that can serve as the CHA team? Who are key stakeholders? Make sure members know expectations of involvement

9 Set Your Timeline What is the target date for CHA/CHIP distribution? Is the date driven by a deadline?

10 Determine Your Model Can be one model or a combination of several Needs to be pre-defined either way

11 What factors make a quality CHA? Look up examples Review PHAB requirements Review IRS standards Request assistance from peers who have done the process already

12 Next steps This work plan is meant to be dynamic! Bring it back to session 2 Be prepared to report progress or lessons learned (if has been completed)
