1 Daily Planners Quality Lifestyle Support Enhancing the Lives of Individuals

2 The daily planner is the form that support workers complete to plan their day/hours with the client/s. Staff MUST sit with each client and discuss the activities for the day (General shift duties are a given and must be carried out. They are not required on this planner). General activities such as meals, medication, showers, personal care etc. are not required on this form either. DAILY PLANNERS The daily planner is completed by the support worker with the input of each relevant client. All daily planners are to be completed within two hours of starting work. The daily planner needs to incorporate and take into account the following: Appointments the client needs to attend (as per client diary) Client recreational/vocational activities All information is to be printed in a clear and legible format. This is a step-by step guide on how staff are to complete a daily planner.

4 STEP 1: CLIENT’S SURNAME & FIRST NAME This is where the client’s surname and first name is entered. If you are working with more than one client, then a separate daily planner needs to be completed for each individual client. It is located at the top of the form, as circled below.

5 STEP 2: ADDRESS This is where the worker enters the address of the client that they are working with. DO NOT enter the unit or street number. EG: Unit 3/1234 Bland St would be entered as Bland St. It is located at the top of the form under the Client Surname and Client First Name fields, as circled below.

6 STEP 3: DATE This is where the actual date is printed. It is to include the day, month and year. EG:12 June 2010 12 Jun 10 13/1/11 13/06/2011 It is located next to the “Day” field, as circled below. STEP 4:DAY This is where the actual day is printed. EG: Friday It is located below the “Client’s Name” field, as circled below

7 During each of the below sections, staff will have standard rostered duties, EG: cleaning, meal preparation etc. However, on each and every day, opportunities exist for activities external of usual daily life. These opportunities need to be extended and discussed with each client. To gain ideas of these activities many tools are available. EG: Local papers, notice boards, clubs, internet etc. Activities such as accessing the community for a variety of pleasurable events such as going for walks, fitness activities, planning for additional out of the ordinary home activities are all viable options. Painting a weathered chair can be an enjoyable activity that produces a result, self-esteem and pride. However this requires planning including the costs of the activity, such as purchase of sandpaper, paint brush etc. This may require transportation to the hardware store, the bank etc. All of this is considered planning. Questions that could be asked to the client: Is there anything that you have been thinking about doing that we could plan to do today? What do you like to do for fun? Let look through the local paper and see if there are any activities that you would be interested in doing today or this week. If we do this activity, what are the things we need to plan to make this happen? Do we have the time, materials, money etc.

8 STEP 5: MORNING In this section, the support worker, after discussions with the client, prints what the plan is for the morning. It is to include times and what is planned to done. It is located below the day and date fields, as circled below. EG:

9 IPP Aligned You will note that there is a Heading titled “IPP Aligned” in the “Morning” field which have with Yes and No boxes, as circled below. If the activity is part of the client’s IPP (Individual Personal Plan) then the relevant box needs to be ticked.

10 STEP 6:AFTERNOON In this section, the support worker, after discussions with the client, prints what the plan is for the afternoon. It should include times and what is planned to done. It is located below the “Morning” field, as circled below. EG: IPP Aligned As per previously discussed

11 STEP 7: EVENING In this section, the support worker, after discussions with the client, prints what the plan is for that night. It should include times and what is planned to done. It is located below the “Afternoon” field, as circled below EG: IPP Aligned As per previously discussed

12 STEP 8: NOTES The case notes section is located below the “afternoon” section of the form, as circled below. This section is where generalised behaviours are printed. If there has been an incident with the client then it should be printed to refer to an incident report. This section is also utilised to explain if the plan did not proceed as written. EG:

13 STEP 9:SUPPORT WORKER SIGNATURE This section is where the support worker signs their name. It is located below the “Notes” section of the form, as circled below. STEP 10:SUPPORT WORKER’S NAME This section is where the support worker prints their name. It is located below the “Notes” section of the form, as circled below.

14 STEP 11:COORDINATOR’S SIGNATURE This section is where the Coordinator prints their name, signifying that they have read the Daily Planner. It is located below the “Support Worker’s Name” section of the form, as circled below. STEP 12:DATE This is where the Coordinator prints the date that they have signed the “Daily Planner”. It is located below the “Coordinator’s Signature” section as circled below.

15 INDIVIDUAL HOUR DAILY PLANNERS Support workers who are not working shifts with clients are still required to complete daily planners. The daily planner should be completed as early as possible, so that maximum benefit can be achieved by the client for the time allocated. The support worker needs to cross out sections of the daily planner “Morning”, “Afternoon” or “Evening” sections if they have not worked those times. EG:
