1 Introduction on Integrated Home Care Services (IHCS) Mrs Yuk Kwok Lai-sum Chief Social Work Officer (Elderly)1 Social Welfare Department 21 and 22 July 2004

2 2 Mission and Aims of IHCS actualize the concepts of “ageing in place” and “continuum of care” provide holistic care and support service to the elders, disabled and needy families living in the community according to their individual needs

3 3 Target Groups Elderly persons: People aged 60 or above. Disabled persons: People with disabilities including those with mental handicap, physical handicap or mental illness. Individuals and families with social need: Individuals who are suffering from chronic illness or ill health, or families which encounter difficulties arising from desertion, sudden illness, hospitalization, imprisonment, death, etc.

4 4 Categories of Service Groups After re-engineering home-based services, the classification of cases is as follows: (1) Frail Cases: Elderly persons: cases who suffer from moderate to severe level of impairment, and require a comprehensive package of service; People with Disabilities: those severely physically handicapped persons who require a comprehensive package of service.

5 5 Categories of Service Groups (cont ’ d) (2) Ordinary Cases: Cases who suffer from no to mild level of impairment or disability, or those cases that are assessed at moderate or severe level of impairment but require only personal care and home-making service.

6 6 Scope of Service (for frail cases) Service Operator provide and arrange the following services -- Care management; Nursing care; Personal care; Restorative and maintenance rehabilitation exercises; Short-term/ part-time centre-based day care service (PWDs can obtain such services from rehabilitation service units); Support Service;

7 7 Scope of Service (cont’d) Carer support service; Home respite, day respite and residential respite service (PWDs can obtain such services from rehabilitation service units); Out-of-hour emergency support; Environmental risk assessment and home modifications; Home-making service e.g. household cleaning, changing of bed sheets and pillowcases, purchase of necessities, laundry, meal preparation, etc.; Provision of meals; and Transportation and escort service.

8 8 Fee-charging It is based on a sliding scale with reference to individual or family income.
