2 SREE NARAYANA MISSION (Singapore) The Mission was named after Sree Narayana Guru (1855 – 1928), a spiritual leader and social reformer from Kerala whose teachings emphasized social equality, the universality of humanity and moral qualities such as compassion. Deeply influenced by Sree Narayana Guru’s teachings, the founders first started the organization to propagate these values and ideas. Since 1948, Sree Narayana Mission Home was registered as a charity organization. Sree Narayana Mission (SNM) Home for the aged sick, located in Yishun was officially opened in 1994 by President Tony Tan. We have 224 beds for bed & wheelchair bound residents. Under the umbrella of SNM, Singapore We have:  Meranti Home @Pelangi Village, destitute home for 250 mentally burnt out male residents, located at 6 Buangkok Green.  SNM Multi Service Centre @Yishun for 20 moderately disabled elderly needing continuity of care in health maintenance  SNM Multi Service Centre @Woodlands for 30 persons with dementia and 20 moderately disabled elderly needing continuity of care in health maintenance. It is a centre-based programme developed by the Agency for Integrated Care (AIC). The service level of this program is between Social Day Care and Day Rehab to promote “ageing in place” for frail and physically disabled elderly in the community. What is the objective of Enhanced Eldercare Programme?  To right-site and address existing service gap by providing access to continued care needed by the frail and/or physically impaired elderly.  To maintain and/or improve the general, physical and social well-being of target clients.  To provide support and respite to family/care givers of target clients. What does our Programmes provide?  Personal Care Services.  Basic Nursing & Health services.  Basic Case Coordination service.  Physical activity, exercise programme, recreational activity. What are the admission criteria for our Programme? 1. Clients requiring day custodial care services including (but not limited to):  Elder sitting.  Assistance with activities of daily living and personal care needs and/or  Meals preparation (including preparation of special diet) and/or  Administration of medication. 2. Shah MBI scores of < 75 points (i.e at least of moderate level of ADL dependency). 3. Requires maintenance exercise program to maintain current physical status or prevent deterioration (i.e low/no rehabilitation potential significant physical improvements). What is dementia? Dementia is a term that is used to describe a collection of symptoms including memory loss, problems with reasoning and communication skills, and a reduction in a person’s abilities and skills in carrying out daily activities such as washing, dressing, cooking and caring for self. What support is available? Support for the person with dementia Day Care Centres - The number of days a person can attend and the types of centre will vary from area to area. Different areas have different types of support available. For more information, please call 63625152 Support for family carers Carers Groups – a great way of meeting other carers and sharing experiences. Carers Groups also enable you to meet “others in the same boat”. Care giver Training is also available. Service Provisions Program Day: Monday to Friday (weekends & PH exclusive) Centre Operating hours: 8am – 7pm (11 hours) Components in Program fees includes Prescribed Therapies, 1 Breakfast, 1 lunch, 1 dinner and Tea snacks. Supervision of medication taking. Monitoring of health status. Charges for Program fees Enhanced Eldercare Program : $49 per day before subsidy. Dementia Day Care Program: $55 per day before subsidy Transportation Charges per round trip is according to distance. Door to door service: $5 will be charged if required. Special clinical consumables Used for dressing or feeding eg. special feeding tubes or special dressings are NOT INCLUDED in program fees.
