1 DAMAGE CLAIM NUMBER REQUEST PROCESS 12/8/10 FEMO PD info rev 09/2011

2 INCIDENT OCCURS – The Operations Center assigns a PTC Event Number (CADS Incident Number) when notified of the accident or incident. PSP Report is generated at the same time and logged with the PTC Event Number.

3 REQUEST FOR WBS ELEMENT Send Email to PDClaims email address: Copy the appropriate District Damage claim email address (D1_PDClaims, D2_PDClaims, D3PDClaims, D4_PDClaims, D5_PDClaims) By using this email address in all correspondence and replies the appropriate District (Superintendent, Operation Manager, and District & Production Clerk) and Central Office Maintenance Personnel will be copied.

4 FEMO PD CLAIM INFO REQUEST FOR WBS / PD CLAIM NUMBER -Send Email to PD Claims email address: PDCLAIMS (this is risk mgmt department) -Copy the appropriate FEMO District PD Claims mailbox/email: (FEMO District 1 Property Damage Claims, FEMO District 2 Property Damage Claims, FEMO District 3 Property Damage Claims, FEMO District 4 Property Damage Claims, FEMO District 5 Property Damage Claims) By using this email address in all correspondence and replies the appropriate FEMO personnel and central office personnel will be copied.

5 FEMO WBS REQUEST FORMAT Include the following information in the Subject Line of the e-mail FEMO District # WBS Request, Date, Event Number Include the following information in body of the e-mail Requesting Location – FEMO DISTRICT LOCATION Event Number PSP Number Date of Incident Time of Incident Milepost/direction or location – Including Route Indicator (whatever applies) Damage Description: Be brief with your damage description but include enough detail to define the damage including roadway cleanup, etc. Similar information should be in the work order text but with more detail. Photographs of damage are to be attached with the request.
