4 wellness profile Health Screening (on-campus or from your doctor) Wellness Advising Session – solidify behavior change with one-on-one support Personal Wellness Plan – take what you’ve learned and develop a plan Complete SHALA & the Personal Wellness Profile and receive $200 (taxable)

5 Fitness Assessment (Free) Personal Training Sessions (Free) Stress & Resiliency Workshops (Free) Nutrition Workshops (Free) Healthy Living Classes (STAP/Free) Physical Activity ($20 HIP/DAPER classes - 2 per qtr or self-reported activity) BeWell Walkers (Free) Physical Exam (Co-pay) Wellness Advocate (Free) Complete SHALA & 5 of the 9 BeWell Berries and receive $100 (taxable) bewell berry activities

Increased flexibility to move through the program however you like (after SHALA) 8 hours of well time to allow you to engage in the program BeWell Numbers Biometric Screenings provided by HIP/BeWell staff Financial rewards distributed for completion of: SHALA + Personal Wellness Profile ($100) SHALA + BeWell Berry Activities ($200) $100$200

7 You Spoke and We Listened! Physical Activity Berry (earn berry credit for regular exercise, not just group fitness classes done on campus!) Maximum of 2 DAPER/HIP group fitness classes at $20 discounted rate, acceptations BeWell Walkers New structure for Stress and Resiliency and Nutrition Workshops Maximum of 2 self-report Berries (Physical Activity, Wellness Advocate, Physical Exam)

8 well time Starting January 1 st … You can convert 8 hours/calendar year of sick time into well time! *In other words, full-time employees who have accrued sick time may use it to participate in approved wellness activities, up to a maximum of 8 hours each year.

9 BeWell is more than a program… 23.2% of you have started using alternative transportation 37.5% of you report to have increased knowledge of your risk factors 80.8% of you incorporated more physical activity into your daily lives 44.4% of you support and value wellness in others around you *Responses taken from 392 responses of a random sample survey.
