2 Purpose of this training: Provide a brief review of the citizenship/identity documentation requirements for Medicaid.

3 Citizenship must be documented:  Initially  When adding a new person  If a second, third, or fourth tier verification is used, identity must also be documented.

4 Document how citizenship was verified If citizenship verified by a Vital Records match, paste a copy of the match on REMA OR document the date VS was accessed, the name listed, the birth date and birth place

5 Document how citizenship was verified At review, do NOT assume verification is in the record CHECK for the verification You do not need to repeat the documentation at every review, but it is your responsibility to MAKE SURE it has been done.

6 IF a “second tier” verification was used, also document how ID was verified Again, at review, do NOT assume the declaration is in the record CHECK for the verification You do not need to repeat the documentation at every review, but it is your responsibility to MAKE SURE it has been done.

7 Aaron, 9, needs Medicaid. He appears to be eligible for RSM; he’s never received Medicaid. We are able to locate his record on the Vital Records site. Will his parents need to provide his birth certificate to verify citizenship? No Will his parents need to sign a Declaration of Citizenship for him? Yes, this will both “declare” his citizenship and provide proof of his identity Recall this example from the basic Citizenship/ID GPS:

9 Clearly document how citizenship is verified. At review, CHECK to be sure the verification and documentation are in the record. Document the date the Declaration of Citizenship was signed. At review, CHECK to be sure the DOC is in the record. Three important points: If a “second tier” document, such as the Vital Records match or a birth certificate, is used to verify citizenship also document how ID was verified. At review, CHECK to be sure verification of ID is in the record.

10 ALERT....ALERT.... ALERT Once the Declaration of Citizenship has been signed to verify the identity of a child under 16... DO NOT verify that individual’s identity again REGARDLESS of current age. Even if the case closes and a new application is taken, do not verify again. DO NOT REQUEST A PICTURE ID ONCE THE CHILD REACHES 16! The only exception (when we would get new verification) is if the case record is lost or the verification can’t be located.

11 Citizenship verification is PERMANENT and must be filed on the left with the permanent verification. The Declaration of Citizenship is PERMANENT and must be filed on the left with the permanent verification. To repeat: We must check at EVERY review and especially if the case is selected for an ASO or PERM review to make sure THE CITIZENSHIP VERIFICATION IS IN THE CASE RECORD.
