Quarterly Reports  We will begin collecting quarterly reports on a regular basis.  Please submit by the following deadlines:  July 20 th – SY 2014-2015 Quarter 4 report  October 20 th – SY 2015-2016 Quarter 1 report  January 20 th – SY 2015-2016 Quarter 2 report  April 20 th – SY 2015-2016 Quarter 3 report  July 20 th – SY 2015-2016 Quarter 4 report

7. Open Enrollment for Medical and Drug Prescription Plans  Per DOE Memo dated May 8, 2015  “Notice of Open Enrollment for New Medical and Prescription Drug Plans Being Offered to Part-time, Temporary, and Casual Employees”  ALL part-time, temporary, and casual employees whose employment start or hire date commences on July 1, 2015 and thereafter, will receive notification of the new EUTF plans for School Year (SY) 2015-2016  Memo dated May 8, 2015 entitled, Open Enrollment for New Medical and Prescription Drug Plans Being Offered to Part-time, Temporary, and Casual Employees;  EUTF’s memo dated May 2015 entitled, Medical and Prescription Drug Plans Being Offered to Part-time and Temporary Employees; and  ACA Declaration Form

8. Site Monitoring  As mentioned on A+ Timeline and in the Operations Manual, Districts are required to monitor ALL school sites at least once a semester.  Arrange with the school site when you will be monitoring  Use A+ Visitation Checklist when at the school site

10. District Items  A+ Operations Manual  A new revised version is currently in the process of being edited and approved  Program Specialist will inform the Districts when the Manual is officially released  Manual will be available online

11. District Items  A+ Attendance  Due to after school service programs (such as 21 st CCLC, after school tutoring, etc.), many students do not report first to A+  Hawaii-East developed a form that could be used in these types of situations

13. Breakdown for SY 2014-2015 – Total Funds Collected Private Providers (67%) DOE (33%) District Offices (72%) Projected Expenses for 2014-15 4 th Qtr. (15%) State A+ Office (2%) Accommodations (11%) **Subsidies are paid for by DHS. Any remaining balance is returned to DHS.
