The scores If your overall score is above 80 Lynne will check it - 55 or higher there is reason for optimism If your overall score is 55 or higher there is reason for optimism If your score is below 35, we usually advise that you suspend the project and concentrate on improving the likelihood that it will sustain For all; work on improving the two factors that have the biggest potential Re score in about 6-8 weeks

Learning through Sharing Katherine Foulkes Waikato dhb

What you need to do Identify the two factors which are of greatest concern- where your actual score is a long way from the potential score. These are the areas that will require work. Identify the two factors which are the most positive – where your actual score is nearest to the potential score. These are the areas you should celebrate, maintain and learn from.

If you get stuck Let me know!

Baseline Measures AIM CHANGE MEASURE RAPID CYCLE IMPROVEMENT Langley et al. “The Improvement Guide: A Practical Approach to Enhancing Organizational Performance.” What are we trying to accomplish? How will we know if a change is an improvement? What changes can we make that will result in improvement? PLAN DO STUDY ACT

Initial thinking Work through the Model for Improvement, Aim, Change and Measure. Predict what you think is right for each of these. Use Capture as a baseline…..what is the experience now What are the additional data that you can also use for example; number of complaints, number of falls, waiting time, hospital readmissions?

For example if you work on this around 2 days a week you have around 16 days, if you work on this 1 day a week you have around 8 days. Webcall- Thursday 30 th January 2014- Reviewing your measures.
