In order to request a background check, you must fill out form I-783.
Please make sure to include your complete mailing address and telephone number and/or address.

5. Fingerprints
After filling out the application form, you must obtain your set of fingerprints.
A standard fingerprint form (FD-258) will be provided by the fingerprinting agency.
Depending on where you live, you must either go to:
Local law enforcement agencies like the Sheriff’s office or Police Stations.
Privately owned locations which have Live Scan systems.

The fee for this service is $18 and may be paid using one of the following options:
