1 ABRAZO SCHOOL-BASED HEALTH CARE CLINIC AT CARTWRIGHT SCHOOL DISTRICT NO. 83 Arizona School-Based Health Care Council Annual Meeting November 2, 2012

2 WHO WE ARE: We are an urban school district in the heart of Phoenix in an area known as Maryvale. We service over 19,000 students daily 20 Schools consisting of: 4 = k-8 4 = middle schools 12 = elementary Median income per family: $35,000 (per family, Maricopa County) Percentage of students on free or reduced lunch: 81.55% (September 2012) Home values: $33,260 (October 2012, Maricopa County Elections) Number of foreclosures: 1,315 (September 2012, City of Phoenix ) 2

3 WHY? Proximity/convenience to members of our community. We are very pleased to host the clinic 3 days a week (Monday, Tuesday & Thursday 8:00 a.m. – 4:30 p.m.). Keep our clinic busy: make our parents aware, train our staff to ensure the clinic stays busy. 1,117 students medical needs served since inception (September 2009). Reduce school absenteeism & improve academic achievement: Over 80% of children seen, return to class allowing them to focus on learning. Reduction in absenteeism: yes (5.1%) (district data) Retain & increase student enrollment. 3

4 HOW? Propose to Executive Team ABRAZO draws up School-based Clinic Agreement Provides Certificate of Liability Insurance Attorney review Find a centralized space that provides a separate & secure entrance. Coordinate with ABRAZO to ensure space is sufficient for their needs. Obtain Governing Board approval Allow ABRAZO staff access to facility. 4

5 GOVERNING BOARD ENGAGEMENT: Explain why it is critical to underserved communities. Access Awareness Provide primary medical care, management of chronic illnesses, and some dental services. Keep your Governing Board informed about the clinic: Provide statistics Refresh their memories about the services provided Remind them why it is important to provide this service to our community. 5
