2. INTRODUCTION  PURPOSE:  REVIEW GUIDELINES FOR BOOK CERTIFICATION  REVIEW REQUIRED DOCUMENTS FOR A SUCESSFUL BOOK CERTIFICATION  PROVIDE A CLEAR UNDERSTANDING OF HOW TO ORGANIZE YOUR TEAMS BOOKS!

3. REQUIRED DOCUMENTS  2015 MFFCC FOOTBALL/CHEER RULES  Should be placed in first sleeve of book  2015 CERTIFICATE OF COVERAGE  Include copies of additional riders>if required by opposing team  COPY OF COACH CERTIFICATION FOR DIVISION  All placed in one sleeve for division

4. REQUIRED DOCUMENTS CONT’D  10 COPIES OF 2015 CERTIFIED ROSTER  Typed roster ONLY accepted  Handwritten rosters are NOT accepted  ADDING PLAYERS AFTER ROSTER IS SEALED  REFER TO PAGE 4, SECTION 3 (ROSTERS) FOR GUIDELINES PRIOR TO ADDING NEW PLAYERS  New roster must be typed or will NOT be re-sealed  Handwritten rosters will NOT be re-sealed AND player is NOT eligible until the new roster is sealed.  Original sealed roster will be compared to new roster to ensure eligibility requirements are met to add player(s).

5. REQUIRED DOCUMENTS CONT  COMPLETED IDENTIFICATION CARD  Ensure legible picture is applied  Picture cannot be more than 6 months old  Picture with game jersey recommended, however, NOT required  Ensure signatures obtain from parent, player(or guardian) and headcoach.  Ensure medical release (on back) is signed and notarized  Place in one sleeve

6. REQUIRED DOCUMENTS CONT  COPY OF BIRTH CERTIFICATE  Highlight child’s name and DOB  2015 PHYSICAL FORM  Can be in same sleeve of Birth Certificate (opposed direction)  Highlight player’s name and DATE physical was performed.  Physicals are required to be dated in 2015

7. REQUIRED DOCUMENTS CONT  Senior Rule 2015:  ALL Senior players/cheerleaders are required to provide legal documentation from current school to validate grade level for fall 2015. Place in separate sleeve.  Acceptable forms of Documents:  2014 Report Card  2015 School Schedule  Information required on documentation:  School name and school year  Grade level for fall 2015  Current Student number

8. IN REVIEW  BOOK ORDER FLOW:  MFFCC 2015 RULES  CERTIFICATE OF INSURANCE  COACHES CERTIFICATION  ORIGINAL SEALED ROSTER (10 COPIES)  SEALED IDENTIFICATION FORM  COPY OF BIRTH CERTIFICATE  COPY OF 2015 COMPLETED PHYSICAL  GRADE VERIFICATION (SENIORS ONLY)
