2 Our Mission…  South Cove Community Health Center is dedicated to improving the health and well- being of all medically underserved in Massachusetts, with a special focus on Asian Americans. 3 A Little History… 1972 - We started in a small building on Harrison Avenue in Chinatown where we provided outreach and referral services. 1976 - South Cove opened its first Medical Clinic at 885 Washington Street as part of the Josiah Quincy Community Center 1994 – South Cove expanded services to 145 South Street, Boston. 1996 – Joint venture with Beth Israel, South Cove opened a clinic at 275 Hancock St, Quincy. 2006 – Relocated from 275 Hancock Street to 435 Hancock Street, Quincy. 2012 – Opened new site at 88 Holmes Street, Quincy. South Cove Community Health Center was founded by the Chinatown community with a mission to serve the medically under-served Asian population. 3

8 Provider Multi-Services: Internal Medicine, OB/Gyn, Pediatrics, Behavioral Health, Dental, Behavioral Health, WIC Services, Nutritional Services, Mammography Screening, Bone Densitometry. We also deliver approximately 360 babies per year. 8

9 We are open 7 days/week…  On call Nurses during closed hours that speak English, Cantonese, Mandarin and Vietnamese  South Cove Patients are rated top in low Emergency Department Utilizations in State  60/1000 preventable ED visits compared with 438/1000 preventable ED visits for other health centers  South Cove Patients are rated top for lowest cost for Massheath  60% lower than other health center patients. 9

10 Our Total Monthly Volume…  Approximately 14,000 Visits/month  Our slowest Month is February – Chinese New Year!!! 10

11  As part of the ACA rollout, South Cove files applications for coverage under the new ACA guidelines (since the online system is in English and Spanish only).  We files approximately 25,000 applications over the last 12 months.  We have 11 Full Time Certified Application Counselors on-site everyday to help our patients.  We provide translational services throughout the application process which includes picking an insurance plan after ACA approval and then choosing a PCP.  The entire application process can take 2-3 visits per applicant. 12 Patient Referrals  Each month our patients need 2,500 referrals to outside services such as Hospital, Specialist, Imaging.  South Cove has 5 Full Time Referral Specialists that will obtain insurance approval and schedule the appointment for patients.  If the patient needs interpretation services during the appointment, our partner hospitals will also schedule an interpreter to meet with the patient at the time of the appointment.  Each patient will receive a letter with instructions for their appointment written in their own language. 12

13 Hospital Interpreters  South Cove employs 12 Full Time Asian Interpreters at Beth Israel Deaconess Medical Center that meet with Asian speaking patients for interpretation during their appointments.  Interpreters can speak Cantonese, Mandarin, Vietnamese and other languages.  Last year our interpreters translated for over 26,000 patient visits at BIDMC.  Tufts Medical Center also provides interpretation for Asian patients during their appointments. These are assigned internally by the hospital. 13

14 Labor/Baby Delivery Interpreters  South Cove employs three full time interpreters/navigators that help women during their labor and baby delivery.  These staff are paged immediately when one of South Cove’s patients is admitted to the hospital for delivery.  The provide interpretation and assist the patient with all their ancillary needs during the delivery process. 14

15 Cancer Navigators  South Cove’s Foundation sponsors two Cancer Navigators at partner hospitals – BIDMC and Tufts.  These Navigators will meet with Asian patients diagnosed with Cancer and in addition to providing interpreter services, help them understand the entire treatment process and also help them with additional services related to their diagnosis and treatment program.  Last year at Beth Israel Deaconess Medical Center, our navigator helped 175 Asian patients navigate the treatment process. 15
