5 Block 4 - Route Order/Release Number vOptional - DoD use only Not required by CG activities

6 Block 5 - Destination vEnter the name, complete street address, zip code and telephone number of the final destination point where the carrier is to make actual delivery

7 Blocks 6 and 7 - SPLC (origin/destination) vOptional - DoD use only Not required by CG activities

8 Block 8 - Origin vEnter the name, shipping address and zip code where the shipment is picked up by the carrier

9 Block 9 - Consignee vEnter the name, complete street address and zip code of the department, activity, or person designated to receive the shipment - OR - “Same as block 5”

10 Block 10 - GBLOC (consignee) vOptional - DoD use only Not required by CG activities

11 Block 11 - Shipper vEnter name, address, and zip code of the shipping activity authorized and responsible for the shipment - OR - “Same as block 8”

21 Block 21 - Pickup Serv. Furnished/Vehicle Fully Loaded vThe shipper checks the appropriate box and enters his/her initials, when applicable

22 Block 22 - Carrier’s waybill/freight bill no. vEnter the carrier’s waybill or freight bill vNormally completed by the carrier’s agent prior to billing for transportation charges

23 Block 23 - Stop this shipment at/for vComplete this section when there are stop offs enroute to partially load and/or unload any freight

24 Block 24 - Furnish information on car/truckload/container shipments vFurnish trailer number, rail car number, or container information vIf there is more than one truckload, furnish info for each: 4trailer/truck 4type vehicle used, e.g. trailer, lowboy, etc. 4exclusive use request, if applicable 4partial loading/unloading point, etc., if applicable

25 Block 25 - Carrier’s pickup date vThe carrier enters the date the shipment was picked up vThe GBL is legally in effect and the carrier assumes responsibility for the shipment on the date in block 25

26 Block 26a - Signature of agent vThe carrier’s agent/driver who picks up the shipment should manually sign this block

27 Block 26b - Per vIf agent’s name is signed by an authorized representative, the initials of the representative must appear in block 26b
