6 Role of the EMS Board The EMS Board is regulatory and advocacy Board that: administers and enforces the provisions of the Ohio Revised Code (ORC 4765); makes recommendations to the General Assembly on legislation to improve the delivery of EMS; establishes standards for the performance of EMS providers including certification and training; establishes qualifications & standards of performance for EMS providers, instructors, accredited institutions and CE approved institutions; establishes and maintains a state trauma registry; administers a grant program for training, research & equipment purchase; maintains an EMS incidence reporting system; maintains a statewide EMS plan; and serves as a statewide clearinghouse for discussion, inquiry, and complaints concerning emergency medical services. I will let you glance at what the role of the board is as shown on the this slide. There are 20 members of the Board and they represent various professional organizations across the State. 19 of those members are appointed by the Governor and 1 by the Director of Public Safety. Bill Vedra is the Public Safety appointee, Bill is also the Director of Homeland Security for the State of Ohio. The Board members are given travel expenses only and they donate an enormous amount of time to the state and their communities. Board members and Bio’s are posted on the web page 6

10 Role of the FFSI Committee
Advises & counsels the EMS Executive Director on: establishment and maintenance of fire service training programs for positions of any fire training certification level; classification and chartering of fire service training programs; and training curriculum, certification examinations, training schedules, minimum hours of instruction, attendance requirements, required equipment and facilities, basic physical requirements, and methods of training for all persons in positions of any fire training certification level. The Fire committee plays a very important role in the Fire community and have been the catalyst for some of the most important changes ever made for Fire in Ohio.  HB 401 has catapulted Ohio Fire head and shoulders above most states in the nation requiring continuing education and renewal of certifications. No longer a lifetime certification! Had 90,000 certified FF’s in Ohio, some were dead, retired, moved out of State, with new cards Ohio now stands at 53,000 certified FF 10

The Medical Director directs the Executive Director and advises the EMS Board with regard to adult and pediatric trauma and emergency medical services issues. Dr. Cunningham is very active throughout the Nation and State and provides presentations on many topics on a local, State and National level 13

Administrative arm of the State EMS Board The Ohio Administrative Code (OAC 4765) provides guidance on how to implement EMS and Fire services. The Division develops and enhances a statewide, comprehensive, systematic response to injury, illness and fire, primarily through education and training, certification and strategic planning. We have 26 employees for the Division and they do an incredible amount of work. We have a very efficient system that helps serve 43,000 + EMT’s and 54,000 + Firefighters. We serve the EMS community in the areas of Certification, Testing, Fire, Grants, Education Accreditation, Data and Research, Investigations and EMSC 15

CE for firefighter must be approved through your local Fire Chief or local Fire Charter Program Director For expiring in 2010 – 18 hours required For expiring in 2011 – 36 hours required For expiring in 2012 – 54 hours required Shortened cycles, CE from 1/24/2008 to current expiration date on fire certs counts for CE - NIMS acceptable, as is FEMA, National Fire Academy 19

Minimum 15 CE hrs within 3-year certification cycle Must include at least 1 hour in the following topics: Patient Assessment; CPR; Airway & Oxygen Administration; AED; Illness & Injury Management; Trauma Issues; Anaphylaxis & Patient Assisted Epinephrine Administration OR Ohio First Responder Refresher Course OR Current registration with NREMT at the First Responder or equivalent level OR Passing score within three attempts on an exam approved by the board. This exam may only be taken during the last six months of a First Responder's current certification period EMS CE – starting with 1st responder No matter the level of cert all levels have 4 options to renew CE Refresher and CE, NREMT + Ohio Trauma Triage Protocol, Test in lieu of 25

Minimum 40 CE hrs within 3-year certification cycle Must include at least 6 hrs pediatric, 2 hrs geriatric and 8 hrs of trauma issues including triage and transportation protocols approved by the board OR Completion of a 30-hr Ohio EMT-B refresher OR Current registration with the NREMT at the EMT-B or equivalent level and completion of trauma triage and transportation protocols approved by the board OR Passing score within three attempts on an exam approved by the board. This exam may only be taken during the last six months of an EMT-Basic's current certification period 4 ways! Ohio EMT-Basic – remember NREMT is a separate card, 2-year cert cycle, with separate renewal requirements check for their requirements 26

Minimum 60 CE hrs within 3-year certification cycle Must include at least 8 hrs pediatric, 4 hrs geriatric and 8 hrs of trauma issues to include triage and transportation protocols approved by the board OR Ohio EMT-I refresher program which satisfies 40 hrs of CE plus 20 hrs of additional CE OR Current registration with the NREMT at the EMT-I or equivalent level and completion of trauma triage and transportation protocols approved by the board OR Passing score within three attempts on an exam approved by the board. This exam may only be taken during the last six months of an EMT-Intermediate's current certification period Ohio EMT-I requirements – 4 ways to renew 27

28 CE Requirements - EMT - Paramedic OAC 4765-17-02
Minimum 86 CE hrs within 3-year certification cycle Must include at least 12 hrs pediatrics, 4 hrs geriatric, 6 hrs of emergency cardiac care (may be satisfied by completion of AHA or ASHI ACLS course) and 8 hrs of trauma issues to include triage and transportation protocols approved by the board OR Completion of an Ohio EMT-P refresher course which will satisfy 48 hrs of the required CE plus 38 hrs of additional CE OR Current registration with the NREMT at the EMT-P or equivalent level and completion of trauma triage and transportation protocols approved by the board OR Passing score within three attempts on an exam approved by the board. This exam may only be taken during the last six months of an EMT-Paramedic's current certification period Trauma triage transportation requirement for Ohio Basic, Intermediate and Paramedic is now documentation of that training and it is either on-line from our web page or the classroom version 28

29 CE Requirements- EMS Instructor
OAC During the preceding 2 years has provided 16 hrs of instruction through an EMS accredited training program or Approved CE training program AND Complete 6 hrs of instructional CE as approved by the Board The total number of CE hours have not changed. What is changing is that all Instructors will be moving to a three year card. 6 hrs methodology is how to teach / Accredited or Approved Training Program can provide this CE for Instructors 29

OAC During the preceding 2 years has provided 16 hrs of instruction through an EMS accredited training program or CE training program AND Complete 6 hrs of instructional CE as approved by the Board EMS Instructors and Assistant EMS Instructors can teach the initial training programs at the level of their certificate to practice / CE Instructors can only be the stand alone instructor for CE or Ohio refreshers from Accredited or Approved training institutions CE Instructors cannot teach initial training at Accredited Institutions as a stand alone instructor 30

40 CE For First Responders and EMTs
EMS Board accepts CE credit awarded for portions of an initial certification or licensing course when: Course work is related to EMS operations or patient care and CE credit is issued by the sponsoring entity. ● Portions of an initial certification or licensing course may be counted towards CE credit when: The initial training meets the requirements set forth paragraph (B) of this rule and The sponsoring entity issues written documentation 40

41 CE For First Responders and EMTs
EMS Board accepts CE credit awarded for teaching through an EMS approved or accredited institution when: Prior approval received from program director and medical director CE credit for lesson is earned once within a 12 month period. ● Continuing education credit for first responders and EMTs may be obtained from teaching EMS continuing education through an EMS accredited or EMS approved institution when the following applies: All courses shall have prior approval from the program director; (2) A first responder or EMT may count a presentation for continuing education hours only once during a twelve month period; (3) The number of contact hours awarded to the EMS instructor, Asst EMS Instructors, CE instructors, or guest lecturer for a presentation shall be equal to the number of contact hours established for the course. (a) Contact hours can only be awarded for that portion of the course presented by the EMS instructor, Asst. EMS instructor, CE Instructor, or guest lecturer. (b) Continuing education contact hours may be awarded for additional portions of the activity the EMS instructor, Assistant EMS instructor, CE Instructor or guest lecturer attends (c) Contact hours shall not be awarded for time spent in the development or preparation of a course or presentation. 41

42 CE Certificate of Completion
Must be issued by a provider listed in OAC (A)(4) Include total CE hours awarded and Specific breakdown for pediatric, geriatric, trauma and cardiology hours Class dates must be within certification cycle A certificate of completion should be obtained whenever you complete CE for credit towards your recertification. Certificate of completion should contain total hours for class and specialty hours for class. Explain geriatric trauma total class hours are 3 and you get credit for 3 from geriatric and 3 from trauma (after making total) do not get 6 hours towards total only 3 hours towards total 42

43 Division of EMS EMS and Fire Audits
10% of providers that renew are audited the following month by computerized random selection EMS audit occurs the month AFTER the new card takes effect May conduct audit at anytime if complaint is received Audits are for the previous three year cycle Certificate of completion needed from an accredited institution, approved CE site or meets OAC for EMS Chief or Fire Charter approval for Fire Tell them not to throw away their certificates of completion when they receive their new cards! Will need certificates of completion on the EMS side to show proof of compliance Problem Area - - Audit Form states that courses like PALS, Heartsaver CPR and ACLS Provider must have both the number of hours for the course and the course outline attached to the copy of the card. We continue to receive only the card which delays the audit process 3. We have had many state that they received their new card in the mail and threw away the certificates of completion for the past three year period – Keep your certs for 3 years past and 3 years present as a minimum, may need your certs for proof for employment throughout your career 43

Do not wait until 3rd year of certification to start getting CE in-line If audited – Respond! Call or with questions or with concerns, or information that might affect your certification card This should be “no worries” if CE was completed as required Audits automatically generated from renewal database by the touch of a button, strictly random! 44

Keep and Address Current with the Division – Highly Important!!! If we can’t notify you, you can’t respond! Be honest! If you didn’t complete, say so, don’t forge, or misrepresent anything The EMS Board will work with you to successfully complete an audit If we can’t find you, you will not be notified you are expiring / s , put in secondary in case primary fails, maybe use department for secondary address I know many chiefs say the card belongs to the individual and they are not tracking or watching that card, and that is correct the certification is the responsibility of the individual to make sure they stay certified but I strongly suggest that all departments track their personnel monthly or yearly to make sure you have certified personnel on the street on both the fire and EMS side of the house 45

46 Division of EMS EMS and Fire Audits
Fire audits just started with the February 2010 renewals The Executive Director will monitor the Fire audits for 2010 and make a determination on those that don’t meet or do not respond. With the 1 card 1 expiration, the audit process will be amended in January 2011. Fire started auditing with 10% of February 2010 renewals / New process for fire and those that don’t meet or don’t respond could face disciplinary action by the Committee and Executive Director All certification levels will expire on same date starting in January 2011 and the Executive Director will decide how many levels will be audited and process of determining which levels will be audited 46

47 Trauma Triage Transportation Protocol ORC 4765.40
The law requires the board to adopt rules for education regarding state trauma triage protocols All EMS providers are required to obtain 8 hours of trauma CE in each 3 year certification cycle that includes documented training of Ohio trauma triage protocol. 111 Grants Funding for all grant priorities is determined by the amount of seat belt fines collected annually; approximately $4,000,000 is available each year. The Board-appointed Grants Committee oversees the administration of the Ohio EMS Grants Program to ensure that all grant funds are distributed fairly, in a timely manner, and in accordance with the directions provided by the Ohio EMS Board. Many recent news articles of departments and organizations issued grants by Ohio EMS but failing to use the money for a variety of reasons, this is a reimbursed grant 111

113 Grant Eligibility EMS organizations must also meet one of the following: Must be established or operated by a township, municipality, village, city, county, joint fire district, joint ambulance district, or joint township fire district within the state. OR Must provide services, pursuant to a contract or letter, to a township, municipality, village, city, county, joint fire district, joint ambulance district, or joint township fire district within the state. The purpose of the Ohio EMS grant program is to improve and enhance EMS patient care in the State of Ohio by providing grant funds to eligible applicants. 120 Definition of “substantially utilizes”
An organization substantially utilizes volunteer emergency medical service providers if, on any given date, for the six-month period immediately prior to that date, the organization's daily average number of hours during which the organization used only volunteer first responders, volunteer EMTs-basic, volunteer EMTs-I, or volunteer paramedics, or a combination of such volunteers, was fifty per cent or more of the daily average number of hours that the organization made emergency medical services available to the public. In the law, and for local legal counsel to determine if you meet the State Law for “substantially utilizes”

121 Definition of “Volunteer”
(K) “Volunteer” means a person who provides services either for no compensation or for compensation that does not exceed the actual expenses incurred in providing the services or in training to provide the services. The EMS Board has developed a position paper in regards to staffing requirements. All agencies are encouraged to continue staffing with a minimum of 2 EMTs when possible. The Board recognizes the challenges faced by the volunteer agencies in regards to recruitment and retention of EMS providers, but encourages those services, whenever possible, to continue staffing with a minimum of 2 EMT-Basics.
