Becoming a Patient New Patient Navigator: Arrange visit by acquiring all information and medical records Inform patient on what is needed on first visit Guide patient on what to expect upon arrival

4 Patient Arrival: Front Desk
Front desk receptionists will gather new or updated patient information Direct patient’s to proper area based on type of appointment (i.e. lab, infusion, exam room) Collection of co-pays and direct calls from hospitals, facilities, and other physicians Check-out patient with follow-up appointment, reminder card, and copy of any result of tests performed
