2. This presentation will assist you in the completion of the Form 1006B. The operators agreement to plug oil, gas and service wells within the State of Oklahoma. Date of Last Revision: 2010 Rule Requirement: OAC 165:10-1-10 Explanation of Form: Operator shall agree to plug well(s) in compliance with the Commission rules. This agreement must accompany the operator's elective choice of surety (Form 1006, 1006A, or 1006C). The operator is required to file a Form 1006B with the Conservation Division once every twelve (12) months.
