2. Guidelines  Use EZPO version  Purchasing Limits: - $10,000 for Perkin Elmer, Amersham, and ICN for Fisher (not ICN itself) OR - $2,500 for all other vendors  Requisitions for amounts greater than these limits MUST be approved manually

Guidelines Cont…  Any requisition for radioactive material must be approved by the Radiation Safety Office (RSO) –Failure to comply will result in corrective action, including suspension of permit for the extreme case of repeated offenses.  Once approved by the RSO, the requisition needs to be approved by the user’s department in order to generate an EZPO number.

5. Line Items  CCODE is 5960 for radioactive materials.  State radioisotope name, compound name, and activity. –If all three are not included, the requisition will be rejected.  For contract orders, specify the contract period: start/end dates.
