4. Your family’s medical and health needs will change from year to year. Corporate Care easily accommodates this.

5. Pay for your family’s medical or dental expenses as you incur them. Keep the invoices or receipts, which itemize your health costs, in a spot where you will easily find them.

6. Once a year, or more often if you prefer,……..

7. Fill out our easy to use Claims Form (By hand or by computer)

8. Our claim forms do the following: Tally up your medical expenses, add a 5% admin fee, add $10.00 processing fee, & add GST only on the fees. This is becomes your total health care costs for business expenses.

9. Once you have the total of the health care costs found on the Claim Form, you write a cheque on your business account to Corporate CARE. Or you can use our Credit Card Authorization Form, also found on our website

10. Pack up your claim form, your original medical receipts, and your cheque or credit card authorization form

12. NOW IT’S OUR TURN……. Your part is done!

13. Our CGAs* adjudicate your claims and receipts & check your totals *Certified General Accountant

14. After we’ve processed and checked everything, we make a cheque payable to you personally or direct deposit to your personal bank account, if you provide a void cheque for this

15. We send your original receipts back to you with a Claims Summary to support the expenses for your business records

16. This is usually done within 24 hours of receiving your claim
