5 The Special Diabetes Program for Indians Competitive Grant Program Background: new SDPI funding for 2004-2008  Congress directed that IHS use the new SDPI funds to implement a competitive grant program in AI/AN communities  Congress directed that this funding be used for: Primary prevention of type 2 diabetes “Most compelling complication” (Cardiovascular disease risk reduction) in people with diabetes Evaluation be conducted on program

6 The Special Diabetes Program for Indians Competitive Grant Program  Tribal Consultation on Competitive Grant Program  Area Recommendations Reviewed by the IHS Director  Tribal Leaders Diabetes Committee Recommendations Eligible entities be SDPI grantees Selection consider diversity in Area, region, program size, type of program (IHS, tribal) Strong coordination / evaluation  IHS Director decision

7 The Special Diabetes Program for Indians Competitive Grant Program Competitive Grant Program $ 27.4 million  Grants to 60-70$ 23.3 million entities  Administration, $ 4.1 million coordination, evaluation

8 The Special Diabetes Program for Indians Competitive Grant Program  “Competitive” - Programs competed for available funding  Demonstration projects – implement activities in one of two areas: Primary prevention of type 2 diabetes Implement Diabetes Prevention Program curriculum in individuals at risk for diabetes Community prevention activities Cardiovascular disease risk reduction Implement a clinical, case management, team approach to treating risk factors for CVD to specific target levels in people with diabetes Community awareness activities

9 The Special Diabetes Program for Indians Competitive Grant Program Evaluation  Comprehensive, multi-component evaluation will be conducted throughout the demonstration project activities  Future funding may be affected by evaluation of this activity – very important  Evaluation will be developed through a collaborative process with grantees  Grantees required to collect and report on specific data  National Diabetes Program and Coordinating Center will analyze and report data

15 The Special Diabetes Program for Indians Competitive Grant Program  Diabetes Prevention (DP) Group  Screen for and recruit people with pre-diabetes  Teach DPP 16-session Curriculum in group sessions  Individual coaching on physical activity, weight loss  Community activities  Cardiovascular Disease Risk Reduction (CVD)  Recruit people with diabetes  Intensive case management  Treat CVD risk factors to target goals  Provide education on CVD risk reduction  Community Activities

16 The Special Diabetes Program for Indians Competitive Grant Program Data and Evaluation Requirements  Grantee requirements  Grantees required to collect and report on specific data  Sources: Participant data, RPMS data, IHS Diabetes Audit  Data coordinator and RPMS Site Manager locally – responsive to requests for data  Timely submission of reports, data required  Local review of reports, presentations and publications prior to dissemination – must agree to review within 30 days of receipt; failure to respond treated as concurrence

17 The Special Diabetes Program for Indians Competitive Grant Program Different way of doing business!  Not the same process for non-competitive SDPI  Need to be more structured, rigorous  Importance of initiative  Significant role of evaluation, technical assistance  Absolute need for active participation by all  Need for success
