2 ON TARGET MFH at a Glance Four provider private practice Started working with VITL in March 2010 Signed contract with Medent in Sept. 2010 Staggered EHR modules going live in Jan. 2011 Interfaced with Porter Hospital Lab in Feb. 2011 Attested to Meaningful Use on June 8, 2011 Certified at Level 3 for PCMH on July 1, 2011

3 ON TARGET Selecting an EHR Homework was completed… Worked with VITL Talked with other practices Reviewed demos Narrowed down the list Reviewed contracts and options Signed contract with “best fit” vendor

4 ON TARGET Implementation: January, 2011 It was a busy month! Installed & set-up hardware from the 5th-7th Went live with Practice Management on 10th Pre-loaded EHR based on scheduled patients Rolled out e-Prescribing first Followed with other modules as trained Trained all staff on system during January Sporadic follow up in February Final training on template modification March 17 Documented visits utilizing all templates All doctors trained by early February

5 ON TARGET For Better or Worse… Office was open during training No official “go live” date Added modules until complete Some visits were documented using EHR Remainder with paper charts Gradually increased number of EHR visits By March, all patient visits were in Medent

6 ON TARGET Kept An Eye on the Goal Incorporated elements needed for: Patient Centered Medical Home Meaningful Use Medent built in: Dashboard for criteria Report functionality for disease management Hub for exchange of clinical info (Criteria #14) VITL helped: Security Risk Assessment Guidance on implementation and criteria

7 ON TARGET Meaningful Use Attestation Medicare EHR incentive Examined every criteria Dashboard and reports Used Clinical Quality Measures from PQRS Worked through Security Risk Assessment Created a remediation plan for deficiencies Reviewed the Medicare Attestation User guide Checked the EP Attestation Worksheet Received incentive funds in late July

8 ON TARGET Lessons Learned Document from the start Set up your system to collect data Keep track of progress for criteria Work with EHR vendor for reports Include the entire office Work in conjunction with PCMH DON’T reinvent the wheel You can do it all!
