1 Scholarships and Awards Time to show off what you and your team can do!

2 Service Awards  Now is the perfect time to get your Years of Service in. If the # is 25, 30, 35, 40, 45 or 50 years and more time to send those names in.  Deadline: On or before February 1  (No Late Entries Accepted)  Send to: Kathy Clark @ the FSNA Office. Have you or one of your fellow workers completed 25, 30, 35, 40, 45, and 50 years in School Food Service?

3 100% Membership & Certification  100% Membership Award FSNA recognizes a schools for obtaining 100% membership in FSNA and SNA..  100% Certification Award FSNA recognizes school centers for obtaining 100% certification in SNA.  Deadline: On or before February 1  (No Late Entries Accepted)  Send to: Kathy Clark @ the FSNA Office.

6 SNA Awards SNA’s deadline is March 1, but FSNA’s deadline is February 1.  Read your award carefully make sure you are applying for the correct award.  Heart of the Program  Louise Sublette Award of Excellence  Outstanding Director of the Year  Thelma Flanagan Award

7 How do I apply?

8 Plan  Plan ahead which award you and your team would like to apply for.  Documentation. Keep accurate documentation.  Make sure everyone involved has a “CURRENT” up- to-date FSNA membership card. **This is your responsibility. Post-it-Notes will not be acceptable.  Any award entry with an expired membership card will be disqualified.

9 Helpful Tips  Work with your local S&A chair for ideas and to help in filling out the forms!  Make sure someone checks your work before submitting.  Due dates are critical. Most awards are due on or before February 1. Submit all entries in PLENTY of time! DON’T WAIT FOR THE LAST MINUTE. This causes you to rush and make mistakes.  All awards are mailed to the FSNA office in Tallahassee!  All awards are mailed to the FSNA office post dated before or on February1, 2014.  NO LATE ENTRIES WILL BE JUDGED!

10 Awards Checklist 1. Read rules and criteria for award.  Who qualifies to apply?  Membership and Certification must be current.  Read the entire award entry before applying. 2. NEW IDEAS ONLY! DO NOT RESUBMIT AN ENTRY FROM LAST YEAR. This is not acceptable and you will be disqualified. 3. You must use the current forms that are available on the FSNA website or in the Leadership Book. 4. Be specific in identifying award topic on application. Did you put the name of the award on the front of your book along with your name and chapter.

14 Application Form  Use this year’s form.  Is it filled out completely?  Remember that signatures are VERY important. You will be disqualified if the required signatures are not completed.  Did you include a copy of your current membership card. This is YOUR responsibility.

15 Application Form Cont’d  Is there a page limitation on the pieces of documentation for the award?15 pictures only, this includes clip art, clip art is considered a picture.  Is the award neatly bound in a 1” 3 ring binder.  Does the award entry binder cover have the name of the award, your name, school name, and county, chapter and region on the front.  Have you reviewed the award checklist to make sure that all the necessary information has been included? White 8 ½ x 11 white paper only!

16  Do I have this year’s award application form? You can find it online.  What size notebook will be accepted for the award entry? 1” 3 ring binder.  Is there a page limitation on the pieces of documentation for the award? Yes, Some awards do—check the application. Rember there is a limit of 15 pictures; includes clip art as a picture.  When is the award due? On or before February 1, 2014  Where is the sent to? FSNA office Tallahassee, before or post marked on February 1, 2014. No Late Entries Ask Yourself:

17  Is the award neatly bound in a 1” binder ? Does the award entry binder cover have the name of the award, your name, school name, and county, chapter and region?  Did you include a copy of your “CURRENT” membership card and, if necessary, certification card? NOT EXPIRED  Have you reviewed the award checklist to make sure that all the necessary information has been included?  Has someone checked spelling and grammar? It is suggested you have this done. Ask Yourself:

18 Don’t Forget:  Leave yourself enough time for doing the project, gathering necessary documentation, and putting the award together for it to arrive to the necessary destination on time.  Before submitting your award for judging, have someone else proof read the award entry for content and spelling/grammar.  Also, be sure to have the required signatures on the entry form prior to submitting the entry.

19 Don’t Forget:  You will be disqualified if the required signatures are not completed.  A specific topic can be submitted for ONLY ONE award.  Award entries are judged based on the award criteria and back up documentation only.  No decorative covers. Just plain white paper with your name, name of award, county, chapter and region.

20 Don’t Forget:  All documentation must be specific and marked to indicate what it is being used to backup.  The documentation should be organized in the awards application pattern and placed in a 1” 3 ring binder. Please do not send loose papers. One sided pages ONLY  Divider tabs may be utilized to identify each section in the folder/binder, when applicable.  You will be disqualified if award requirements and guidelines are not followed.

21 GED & Adult Education Scholarship $100  FSNA & SNA Member for at least one (1) year  Permanent SFS Employee for at least one (1) year  Can only receive scholarship once  Fill out Application with District Director or District Supervisor signature  Completion must have taken place within past year  Include copy of Certificate/Diploma of completion

22 FSNA UNDERGRADUATE SCHOLARSHIP Lower Division, $300  Current FSNA & SNA Member  SNA Certified/Credentialed  Permanent SFS Employee for the past two (2) years  College/University Transcript with at least two (2) courses completed, “C” or above Grade Average  Application, one page letter & letters of recommendation.

23 FSNA UNDERGRADUATE (UPPER DIVISION) AND GRADUATE SCHOLARSHIP $1,000  Current FSNA & SNA Member  SNA Certified  Permanent school foodservice employee for at least two (2) years  College/University Transcript – AS degree or AA degree (“C” or above Grade Average)  Application, one page letter & letters of Recommendation

24 Legislative Action Caucus Grant  Who is eligible? Any current member of FSNA who is classified as a food service employee, food service manager, and has never attended a Florida Legislative Action Caucus before may apply.  Up to $300 Offsets the cost of registration, travel, hotel and meals for Florida LAC up to $300.

25 Legislative Action Caucus Grant (cont’d)  Deadline? No later than January 15 to the Public Policy & Legislative (PP&L) Chair  The applicant must write a statement, using 100 words or less, explaining why they would like to attend the Florida LAC and what they hope to take back to their local association.
