Why Focus on Elderly Patients? Multiple chronic conditions Big variation in home/family support Social issues aren’t being addressed Many, many medicines – opportunities for harm Fat complicated medical records – error prone Multiple medical providers Lack of coordination between hospital/ER and primary care providers

Senior Medical Home – Numbers Currently 630 patients enrolled Avg age 82, 74% female 87% Diabetic 90% Hypertension 68% Heart Disease 42% Chronic Pain 56% Psychiatric Diagnosis 263 home visits 3,000 care coordinator contacts 6 ramps built, 3 ready for installation 84 grab bars installed 30 trips to visit patients in the hospital Linking patients with outside resources Educating all about health care reform
