6 What choice of antibiotic could allow a patient to be referred to Home community IV team? Most antibiotics are once daily Occasionally bd courses are given Most commonly used iv antibiotics are: Teicoplanin Daptomycin Ceftriaxone Ertapenem More rarely – ceftazidime, meropenem

7 OPAT – GCS Community iv Service Most frequently treated infective conditions: Cellulitis Osteomyelitis UTIs caused by multiply-resistant organisms Bronchiectasis Referrals from Glos Out of county referrals for pts being repatriated to glos – Oxford (NOC), Bristol, Birmingham, Swindon

8 OPAT – GCS Community iv Service A blood monitoring leaflet has been developed Weekly blood monitoring by the iv therapy team of Routinely – FBC – U+Es and eGFR – CRP When necessary – Antibiotic assays / levels – TDM – CPK

9 Patients need to fit IV pathway inclusion criteria Patients will require a PICC line for IV therapy treatments for more than 10 days Patients must have review and follow up appointment with referrer In-hospital prescription chart, drugs and consumables must be sent home with the patient on discharge OPAT – GCS Community iv Service
