3 Not Qualified for OFLA/FMLA Or exhaust OFLA/FMLA Illness requiring time off (hours or days)—5+ days Follow regular absence reporting requirements with supervisor HR request an accommodation meeting, which will require doctor’s note Meeting with supervisor, employee, and HR to identify reasonable accommodation Must have a return to work form completed by doctor to return to work Complete MESD Paid Leave Form

4 Workers’ Comp Injury at work (IAR and 801) Return to work with restrictions HR contact for light duty position (administrative)— must take assignment Expiration date of workers’ comp Must have a return to work form completed by doctor to return to work Complete MESD Paid Leave Form (wait period)

5 Restrictions: Accommodation Provide HR with doctor’s note HR schedule accommodation meeting Accommodation meeting with supervisor, employee and HR Identify reasonable accommodation and implement plan Must have a return to work form completed by doctor to return to work
