a community-based malaria control project An introduction to the project, prepared by: NatashaTamplin and Jamie Edgerton November 2007 UGANDA THE LAMPS PILOT PROJECT ( L ocal A nti- M alaria P rogramme S upport)

LAMPS project strategies: to MOBILIZE and EMPOWER communities to protect themselves against malaria. to complement existing government anti- malaria initiatives such as: Community Medicine Distributors DDT residual spraying Distribution and re-treatment of nets

Each village organizes its own Fight Malaria Committee (FMC) Each FMC uses participatory techniques to assess their villages competence in protecting itself against malaria Fight Malaria Score Card

Score Card average scores in each domain Village Malaria Competence Profile – (a graphical summary of a villages ability to fight malaria)

Village Malaria Competence Profile Each village designs its own Local Anti-Malarial Programme (LAMP)

Intervention Options for individual village LAMPs Examples: Sewing of local bednets (income-generating activity) Promoting bednet use and retreatment Vector control measures Local protocols for prompt diagnosis and treatment Possible use of natural prophylactics and remedies (Neem or Artemisinin Tea) Pregnant women support groups Training and assisting the Community Medicine Distributors

Periodic re-assessment of Fight Malaria Score Card a higher-level Malaria Competence certification. S afe P rotected M obilized A ware L ow-competence

Underlying Rationale: To develop and validate a community-based intervention strategy that is low cost, and can be disseminated largely through lateral diffusion

Project Overview Lead Agency:Uganda Women Concern Ministry (UWCM) in eastern Uganda Time Frame:2 years Summarized Project Goal: To empower all 56 villages of Bungokho sub- county to protect themselves collaboratively against malaria Total Cost: A$ 370,000

Two Hypotheses for Independent Evaluation An increase in a villages malaria competence leads to a reduced malaria burden LAMPS project interventions lead to an increase in each villages malaria competence

How to test these hypotheses? Compare measures of: –Morbidity –Village Malaria Competence –Associated Factors NOW vs AFTER And WITH vs WITHOUT BUT……

Problems with Clinical Diagnosis Symptoms for malaria are: non-specific overlap with others NOT all fevers = malaria

Challenges in Baseline Survey Design measuring mortality (focus on morbidity) confirming malaria diagnosis v.s. fever (even with Rapid Diagnostic Tests) dealing with government protocols

Research Opportunities The project provides a context to address many research challenges, e.g.: Clinical diagnosis under field conditions Optimal mix of interventions Success factors to foster lateral diffusion ……….

Anyone who thinks that they are too small to make a difference has never tried to fall asleep with a mosquito in the room. -Christie Todd Whitman
