1 Esophageal Sarcomatoid Carcinoma Presenting as a Fever With Elevated Serum Leukocytes
Sun-Kui Ke, MD, Hong-Bing Duan, MD, Ying-Jie Cai, MD, Yun-Peng Cao, MD, Yuan Zhong, MD, Chao Hu, MD  The Annals of Thoracic Surgery  Volume 98, Issue 5, Pages e123-e125 (November 2014) DOI: /j.athoracsur Copyright © 2014 The Society of Thoracic Surgeons Terms and Conditions

2 Fig 1 (A) Contrast-enhanced computed tomography scans obtained 2 days after admission showed a circumferential mass in the middle thoracic esophagus spanning. (B) Endoscopic examination findings show a large mass located 29 cm from the upper incisors that was almost completely obstructive. (C) Pathologic findings from the endoscopy: poorly differentiated squamous cell carcinoma (hematoxylin and eosin staining, ×100). The Annals of Thoracic Surgery  , e123-e125DOI: ( /j.athoracsur ) Copyright © 2014 The Society of Thoracic Surgeons Terms and Conditions

3 Fig 2 (A) Intraoperative examination showed that the tumor did not invade adjacent organs. (B) The tumor was bisected, revealing a mass about 9.0 × 2.5 × 2.5 cm in the esophageal lumen with only a thin pedicel connecting it to the mucous membrane of the esophagus. (C) Pathologic findings from the postoperative specimens, revealing two tumor components: squamous cell carcinoma and malignant spindle-shaped cells (hematoxylin and eosin staining, ×100). 4 Fig 3 The patient presented with persistent fever (blue symbols and line) and high concentrations of serum leukocytes (red symbols). After thoracic esophagectomy, the fever faded and leukocyte serum levels rapidly normalized.
