List the author, title, etc. that would be needed to make the entry in the bibliography of your report.

Bibligraphy Card for a Book will always include these 5 pieces of information: Author (last name first) Title (underlined, followed by a period.) City (followed by a colon:) Publisher (followed by a comma,) Year (followed by a period.)
