Rapid ventricular tachycardia (rate 215/min). The mean frontal plane QRS axis is directed rightward and superiorly. A pure R wave configuration is present in lead V1 and an RS configuration in V6. P waves occurring in 1:1 relationship to the tachycardia complexes are best seen in leads II and V1–2; they appear to be inverted in lead II and thus represent retrograde atrial activation.
