Choose New Ticket to communicate with SFS Give SFS as much information about your question, problem, suggestion as possible Upload files, screenshots, etc. Submit ticket

9 Self Assessment Click on Self Assessment Create New Self Assessment

10 Click here ONLY when ready to send it to the evaluator
Click on the areas of the rubric where you see yourself

11 Teacher’s Artifacts Click on Create New

12 When done, click on” Save and Send” or “Save as Draft”
Put in Title Add your description of the artifact. Click on “Mark As” for the competencies that are indicated with the artifact To attach the artifact, click on “Add New,” give it a Title, select the file, then Upload

13 Professional Development
Click on Create New

14 Either save as draft or save and send to administrator for approval
Give the task a title Fill out this area explaining what the task is Add Professional Growth Points as earned Attach file (certificate, etc) showing that you completed the task

15 Add Professional Growth Points
Choose category Choose date Choose number of hours

16 Professional Points Printable PDF for submission for certification
Staff member may view all activity or individual year’s activity List of activities and hours

17 Observations Observations are numbered
May start new pre-observation form Numbers insides boxes – green is number of times marked in upper two areas; yellow in lower two areas May start post-observation form after a long (formal) has been started Numbers in circles within the rubric represent the number of the observation and where the staff member was marked for that observation

18 Pre/Post-Observation Form
For both pre and post-observation forms, fill in the blanks and send to evaluator

19 Observations To see an observation, click on the observation
Must click on ok to see observation New observations and new comments are shown in red

20 Observation Staff member will see comments by evaluator here and may make their own comments. This is open for a set number of work days May view pdf version The part of the rubric that was marked by evaluator May add attachment here Staff member may see evaluator’s scripting of evidence and code for indicators marked

21 Put in a ticket to SFS!
