5 5 Why Focus on Preventive Care? Dental caries is the most common chronic childhood disease, five times more common than asthma and almost 100% preventable. 6 6 Sealants Are Underutilized There is a major opportunity to enhance the use of sealants and improve oral health. Analysis of the claims of one client shows only 16% of the 26,112 children ages 6-7 who visited a dentist over the last 12 months got sealants. –Best practice would be 100%. –The Department of Health and Human Services goal is 50%.

7 7 How Program Will Work Reports and tracking mechanisms are provided from claims data to help identify your young patients who are at risk and would benefit from preventive services including fluoride and sealants. Ongoing reports generated every 6 months will help you track the progress of treatment including sealant and fluoride placement for your patients, protecting them from tooth decay and improving their oral health for life. By providing practical information that identifies young patients in your practice who would benefit from sealants and fluoride we're helping you ensure that everyone you treat gets the best possible care. Claims Data Report & Tracking Ongoing Reporting Patient Progress Reports

8 8 Your Reports Will Include: Roster: Patients under 21 who were last seen in your practice and are due for sealants and fluoride in the six month timeframe The following information will be included: –Patient name –Date of birth Performance Reports: Follow up reports will contain information about: –members that have been seen for sealants and fluoride –claim detail –members that are still due for services –thresholds met –peer comparison.

9 9 Who is Eligible ? In order to be included in the Patient Report, a patient must: Be eligible for benefits through the Colorado Medicaid program administered by DentaQuest Have been seen by a primary care dental provider at this office at least once Be under age of 21
