2 Improving health care through accreditation ® View an informative webcast Complete a five question quiz Evaluate case studies and corresponding final reports using an electronic questionnaire – Ten consecutive case studies and corresponding reports during each quarter of the activity for a total of four quarters – Electronic questionnaire evaluations submitted with the use of the IAC QI Self-Assessment Tool A summary of each quarterly assessment must be discussed in your facility’s biannual QI meetings Requirements for Participating Physicians

3 Improving health care through accreditation ® Quality Metrics A series of questions to address a minimum of four quality metrics: —Test Appropriateness —Technical Quality Review —Interpretive Quality Review —Report Completeness and Timeliness Additional modality specific metrics may be evaluated: – Radiation Safety (CT and Dental CT) – Report Correlation (Echocardiography) – Case Review (Vascular Testing) Goal: Based on these metrics, achieve and maintain an average overall case study quality score of 95% within one year.

4 Improving health care through accreditation ® Getting Started An Admin User of your facility’s Online Accreditation account will be responsible for initiating and managing MOC activities. An Admin User will be required to: Create and manage quarterly assessments Assign physicians to complete case reviews for the IAC QI Maintenance of Certification Activity – Physicians may participate individually or as a group Provide participating physicians with access to quarterly self-assessment analysis reports Provide quarterly attestations to the physician’s completion of the activity Admin Users will be notified by email when a physician has successfully registered for the MOC activity, as well as when additional action is required.

5 Improving health care through accreditation ® Getting Started Admin Users will be notified when a physician has successfully registered for the MOC activity. The Maintenance of Certification tab will not be visible until at least one physician in your facility has been registered.

6 Improving health care through accreditation ® Getting Started The modalities for which physicians in your facility are eligible to participate in the MOC activity will appear in column to the left.

7 Improving health care through accreditation ® Creating an Assessment This page provides directions on how to manage the MOC Activity.

8 Improving health care through accreditation ® Creating an Assessment Select “Create New Assessment Series” to create a new assessment series. You will then be guided through the creation of the first quarterly assessment.

12 Improving health care through accreditation ® The IAC QI Self-Assessment Tool will prompt you to complete each step in order and continue to provide direction as you manage this activity. Managing an Assessment The current quarter of each assessment series will be highlighted.

15 Improving health care through accreditation ® Entering Case Studies Select Start MOC Process to assign staff to review these cases. Enter ten consecutive cases of the same study type. The case type chosen in the first quarter will be the case type in subsequent quarters.

16 Improving health care through accreditation ® Confirming Case Information Reviewing for accuracy is essential. Select Cancel to return to the Case Log and edit entries. Confirm the entered case information is accurate before continuing.

17 Improving health care through accreditation ® Assign one or more participating physicians to review the cases for this quarter. In subsequent quarters, the participating physicians will be automatically assigned to perform case reviews. Assigning Staff

18 Improving health care through accreditation ® Assigning Staff Once Submit Assignments has been selected, additional physicians can not be added. Assign one or more participating physicians to review the cases for this quarter.

19 Improving health care through accreditation ® The participating physician(s) will receive an email with instructions to review the case studies they have been assigned to review for this quarter. Assigning Staff

20 Improving health care through accreditation ® View the status of assigned case reviews for this quarter. An admin User may remove a physician from the MOC activity as well. Managing an Assessment

21 Improving health care through accreditation ® Managing an Assessment Physicians who have been removed from the MOC activity cannot be added back to the assessment. They may, however, be assigned to participate in other MOC activities.

22 Improving health care through accreditation ® Once the participating physician(s) have completed all case reviews within the assessment, they are required to review a Quality Improvement Self-Assessment Analysis report which includes a quarterly summary and the findings from all case studies within the assessment. Participating physicians and Admin users will be notified by email when the report is available. Viewing the Self-Assessment Analysis Report

26 Improving health care through accreditation ® Attestation At the end of each quarter, an Admin User will attest that the participating physician(s) reviewed the Quality Improvement Self-Assessment Analysis report. Additionally, at the end of the fourth quarter, an Admin User will be required to provide additional supporting documentation.

27 Improving health care through accreditation ® Attestation An Admin User must attest on behalf of all participating physician(s) before the next quarter may be started.

28 Improving health care through accreditation ® Uploading Required Documentation Following the Q4 attestation, the Admin User will be asked to upload: The Facility’s biannual QI meeting minutes documenting each physician’s participation. A summary of the findings from the Quality Improvement Self-Assessment Analysis from the IAC MOC activity. A completed and signed attestation form from the MOC provider (ABP or ABIM) for each participant. Or: A letter of attestation from your facility’s Medical Director stating that the participating physician(s) participated in the facility’s most recent QI meeting, and that the findings from the MOC activity were discussed. A completed and signed attestation form from the MOC provider (ABP or ABIM) for each participant.

29 Improving health care through accreditation ® Awarding MOC Activity Participation Points Participating physicians will be awarded activity points when: The Q4 attestation indicating all participants have reviewed the final Quality Improvement Self-Assessment Analysis report has been submitted. The required documentation has been uploaded. IAC will review all documentation submitted by your facility before participation points are awarded. Participating physicians and Admin users will be notified by email when points have been awarded.
