Jay Larson Manager, Nuclear Safety Information (NSI) Dashboard Office of Environment, Health, Safety and Security Office of ES&H Reporting and Analysis (AU-23) June 14, 2016 Nuclear Safety Information (NSI) Dashboard Presentation for The DOE Operating Experience Committee Webinar 1

Briefing Agenda for the Nuclear Safety Information (NSI) Dashboard Review the purpose of and introduce the NSI Dashboard. Explore NSI Dashboard capabilities online. Discuss possible, future actions involving AU-1. – Establish NSI SME working group and NSI Steering committee. – Engage Nuclear Safety Committee and the Nuclear Safety Coordinating Committee. – Establish positive outlook on data trends and solutions. Avoid compliance approach. 2

Goal and Purpose Nuclear Safety Information (NSI) Dashboard Goal: Facilitate use of the DOE ORPS database for quicker and more in-depth analysis and trending of nuclear safety information. NSI extracts ORPS nuclear safety data to eliminate time consuming searches. NSI displays data in trending/analytical graphic format. Easily accessible and usable. NSI speeds up the process to distill and look at ORPS nuclear safety data. Nuclear safety precursor-type events are weighted and grouped, and weighted categories are presented by PSOs and sites. 3

Nuclear Safety Information Dashboard: Functions Organizes nuclear safety data from ORPS into an easily accessible and usable format, in real time. Nuclear safety precursor-type events are weighted and grouped, with goal to prevent high consequence nuclear events (i.e., leading indicators.) 4

Status: NSI Dashboard has been reconstructed from 2012 development. Using the 2012 developed weighted scoring. Functionality has been added. 5

Added Functionality Includes.. Reporting Criteria Keywords Report Type (Final, etc.) Significance Category Fiscal Year, CY Quarter Total Events High Consequence Events Dual Charts 6

EM chart for total number of ORPS events. 7

This chart shows the weighted scores of nuclear safety events from the ORPS reporting system. The legend on the right shows the 19 groupings of 65 reporting criteria from ORPS that comprise weighted scoring for nuclear safety events. Orange shaded groupings are nuclear events, and blue are nuclear related. Nuclear Safety Controls is second from the bottom of the stacked bars, and is increasing. 8

EM-Wide Nuclear Safety Controls: (1) Technical Safety Requirements (TSRs) violations, (2) Documented Safety Analyses (DSAs) inadequacies, and (3) Criticality deficiencies. [ORPS Reporting Criteria 3A-C.] Little change from 2012 to 2015. 2013 had highest number of events. 9

EM-Wide Nuclear Safety Controls: (1) Technical Safety Requirements (TSRs) violations, (2) Documented Safety Analyses (DSAs) inadequacies, and (3) Criticality deficiencies. [ORPS Reporting Criteria 3A-C.] 32% increase from 2012 to 2015. 110% increase from 2010 to 2015. 10

Next Steps Have briefed targeted mid-managers and SME staff of NNSA, EM, NE, SC and EA. Normalizing the data to manpower (i.e., # events/1000 FTEs). Propose and organize a working group of SMEs and a steering committee of mid-managers to review and form consensus on the weighted scoring. Update documentation of the NSI Dashboard. Make long-term plans to develop weighted scores for non-nuclear events. 11

Example of Weighted Scoring Nuclear Safety Controls 3A(2) Violation of Hazard Category 1, 2, or 3 nuclear facility’s Technical Safety Requirement, Operational Safety Requirement, Limiting Control Setting, Limiting Condition for Operation, Specific Administrative Control, or Surveillance Requirement 2.6 3A(3) Violation of credited hazard control specified in Hazard Category 1, 2, or 3 nuclear facility’s Documented Safety Analysis 1.5 3A(4) Surveillance test performed late in which the structure, system, or component was capable of performing its safety function 1 3B(1) Positive Unreviewed Safety Question (USQ) reveals inadequacy in documented safety analysis 0.8 3C(2) No documented controls available to prevent a criticality accident. An accident has not occurred due to other, non-documented barriers or controls 6.9 3C(3) Loss of one or more nuclear criticality documented controls such that an accidental criticality is possible from the loss of one additional documented control 3.8 3C(4) Deficiency in criticality safety analysis or degradation of a documented criticality control such that adequate controls were not in place for a credible criticality accident scenario 2.5 17
